加减八珍汤改善持续性不卧床腹膜透析患者营养不良及微炎症状态的临床疗效观察

摘　　要：目的:观察和认识加减八珍汤对持续性不卧床腹膜透析患者营养不良、微炎症状态的作用。方法:观察对象为2020年9月—2021年12月在六安市中医院肾病二科就诊的60名持续性不卧床腹膜透析患者,将60名患者随机分为对照组和治疗组,2组各有30例。2组都接受基础治疗,治疗组患者接受基础治疗同时加用加减八珍汤,比较两组患者治疗前后营养不良-微炎症(MIS)评分的改变,观察患者血红蛋白(Hb),血清白蛋白(ALB),转铁蛋白(TRF),前白蛋白(PA),C反应蛋白(CRP),白介素6(IL-6)的变化。结果:与治疗前相比,两组患者治疗后MIS评分均明显降低(P<0.01);与治疗前相比,2组患者治疗后Hb、Alb、PA及TRF均升高,并且治疗组Hb、Alb、PA及TRF相较对照组明显升高(P<0.01);与治疗前相比,两组患者治疗后CRP、IL-6均降低,并且治疗组CRP、IL-6相较对照组明显降低(P<0.01)。结论:加减八珍汤可以改善持续性不卧床腹膜透析患者营养不良、微炎症状态。Objective:To observe and understand the effect of Modified Bazhen Decoction on malnutrition and micro-inflammatory state in patients with persistent ambulatory peritoneal dialysis.Methods:The subjects were 60 continuous ambulatory peritoneal dialysis patients who were treated in the Second Department of Nephrology,Lu'an Hospital of Traditional Chinese Medicine from September 2020 to December 2021.The 60 patients were randomly divided into the control group and the treatment group,2 groups There are 30 cases each.Both groups received basic treatment,while the treatment group received basic treatment plus Jiajian Bazhen Decoction.The changes of malnutrition-microinflammation(MIS)scores were compared between the two groups before and after treatment.Hemoglobin(Hb),serum albumin and(ALB),transferrin(TRF),prealbumin(PA),C-reactive protein(CRP),interleukin-6(IL-6)changes.Results:Compared with before treatment,the MIS scores of the two groups were significantly decreased after treatment(P<0.01).,Alb,PA and TRF were significantly higher than those in the control group(P<0.01);compared with before treatment,CRP and IL-6 in the two groups were decreased after treatment,and the CRP and IL-6 in the treatment group were higher than those in the control group significantly decreased(P<0.01).Conclusion:Modified Bazhen Decoction can improve malnutrition and micro-inflammatory state in patients with continuous ambulatory peritoneal dialysis.
