摘 要:目的 分析接受免疫治疗后发生皮肤毒性的非小细胞肺癌(NSCLC)患者血清中白细胞介素-6(IL-6),白细胞介素-10(IL-10)的表达水平与皮肤毒性严重程度的相关性。方法 选取2019年9月—2021年9月我院发生免疫治疗相关皮肤毒性的NSCLC患者105例作为观察组,55例健康体检者为对照组。分析IL-6、IL-10的表达水平与NSCLC患者的临床特征、各级别皮肤毒性的关系。结果 NSCLC患者的血清IL-6,IL-10水平高于健康人群。血清IL-6、IL-10的水平与NSCLC患者的性别、年龄、吸烟史、病理类型无明显统计学相关性(P>0.05),但随着肿瘤分期晚、ECOG评分升高、发生远处转移,IL-6、IL-10水平明显升高(P<0.05)。根据皮肤毒性分为G1组、G2组和G3组,免疫治疗前各组患者的血清IL-6、IL-10表达水平无明显差异,免疫治疗后G3组的IL-6、IL-10浓度显著高于G1组和G2组(P<0.05)。结论 血清IL-6、IL-10的高表达水平与NSCLC患者高肿瘤负荷相关,免疫治疗过程中动态监测IL-6、IL-10的浓度变化有望提前识别治疗相关皮肤毒性的严重程度。Objective To determine the correlation between the severity of immunotherapy-related dermal toxicity and the levels of interleukin-6 (IL-6) and interleukin-10 (IL-10) in patients with non-small cell lung cancer (NSCLC).Methods Totally 105 patients with NSCLC with immunotherapy-related dermal toxicity in our hospital from September 2019 to September 2021 were selected as an observation group,while another 55 healthy subjects served as an control group.The correlation between the expression levels of IL-6 and IL-10 and the clinical characteristics and dermal toxicity of NSCLC patients was analyzed.Results The expression levels of IL-6 and IL-10 in the serum of NSCLC patients were higher than those in healthy subjects.The expression levels of IL-6 and IL-10 in the serum had no significant correlation with gender,age,smoking history,and pathological type of NSCLC patients (P>0.05).However,the levels of IL-6 and IL-10 significantly increased with advanced tumor stage,increased ECOG score,and distant metastasis (P<0.05).When groups were decided by dermal toxicity (G1/G2/G3),there was no significant difference in the expression levels of serum IL-6 and IL-10 in each group of patients before the immunotherapy,while the concentrations of IL-6 and IL-10 in group G3 were significantly higher than those of group G1 and G2 after the immunotherapy (P<0.05).Conclusion High expression levels of serum IL-6 and IL-10 are associated with high tumor burden in NSCLC patients.Dynamic monitoring of cytokine IL-6 and IL-10 concentrations during the immunotherapy may predict the severity of immune-related dermal toxicity.
