摘  要:目的确定猪急性心肌梗死(AMI)模型中血清缺血修饰白蛋白(IMA)水平变化的时间进程,并探讨血清IMA水平与左室射血分数(LVEF)的相关性。方法采用球囊介导的左前降支永久性闭塞术诱导滇南小耳猪AMI。采用第三代双源CT(DSCT)对AMI诱导前后30 d内LVEF动态变化进行评价,同时测定血清IMA水平,并进一步确定血清IMA水平与LVEF的相关性。结果血清IMA水平在AMI后1 h升高,于6~12 h达到峰值,此后逐渐下降,30 d后恢复至基线值。早期血清IMA水平与LVEF呈显著负相关(r=-0.41~-0.80,P<0.05)。30 d血清IMA水平与各时间点LVEF无相关性(P>0.05)。结论猪AMI模型早期循环中IMA水平与LVEF呈显著负相关,血清IMA水平对AMI不良预后可能具有早期预测价值。Objective To determine the time course of changes in serum ischemia-modifies albumin(IMA)levels and to investigate the correlation between serum IMA and left ventricular ejection fraction(LVEF)in a swine model of acute myocardial infarction(AMI).Methods Balloon mediated permanent occlusion of the left anterior descending artery was used to induce AMI in Diannan small-eared pigs.Dynamic changes in LVEF before the AMI induction and within 30 day after AMI induction were evaluated by using third generation dual-source computed tomography(DSCT).Serum IMA levels were measured at the same time points.We further determined the correlation between serum IMA levels and LVEF.Results Circulating serum IMA levels were increased at 1 h,peaked at 6-12 h,gradually decreased after 12 h,and returned to baseline 30 days after AMI.Early serum IMA levels were negatively correlated with LVEF(r=-0.41--0.80,P<0.05),but there was no significant correlation between 30-day serum IMA levels and LVEF at each time point(P>0.05).Conclusions Circulating IMA levels are negatively associated with LVEF during the early stage of AMI,suggesting that serum IMA level has early predictive value for adverse outcomes of AMI.
