摘 要:目的:探讨利胆清胰解毒汤对急性胆源性胰腺炎大鼠炎症因子、肠道屏障功能及瞬时受体电位香草酸亚型1(TRPV1)、降钙素基因及相关肽(CGRP)表达的影响。方法:将50只SD大鼠随机分为正常对照组、模型组、阳性组、利胆清胰解毒汤低剂量和高剂量组。采用胆胰管结扎术造模,造模成功后,正常对照组、模型组给予生理盐水灌胃,阳性组给予奥曲肽(50μg/kg)、低剂量组给予7.2 g/kg利胆清胰解毒汤、高剂量组给予21.6 g/kg利胆清胰解毒汤灌胃,1次/天,连续7 d。观察各组大鼠胰腺组织病理变化及评分;检测各组大鼠血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-10(IL-10)和白介素-1β(IL-1β)等炎症因子水平;测定内毒素(ET)、D-乳酸(DLA)、二胺氧化酶(DAO)含量以及淀粉酶(AMY)等肠道屏障功能指标;采用Real-time PCR检测大鼠胰腺组织TRPV1、CGRP mRNA表达。结果:模型组大鼠胰腺组织评分明显升高,血清TNF-α、IL-6、IL-1β水平、肠道屏障功能指标、大鼠胰腺组织的TRPV1、CGRP表达明显上升,IL-10水平明显降低(P<0.05);给予药物干预后,阳性组、低剂量组以及高剂量组大鼠胰腺组织评分明显降低,TNF-α、IL-6及IL-1β水平、肠道屏障功能指标、大鼠胰腺组织的TRPV1、CGRP表达明显降低,IL-10水平明显升高(P<0.05),阳性组和低剂量组间比较无显著性差异(P>0.05),阳性组、低剂量组与高剂量组间比较具有显著性差异(P<0.05)。结论:利胆清胰解毒汤可有效降低急性胆源性胰腺炎模型大鼠炎症因子水平,改善肠道屏障功能,调节TRPV1、CGRP表达。Objective:To investigate the effect of Lidan Qingyi Jiedu Decoction on inflammatory factors,intestinal barrier function and expressions of transient receptor potential vanilloid 1(TRPV1),calcitonin gene and related peptide(CGRP)in rats with acute biliary pancreatitis.Methods:50 SD rats were randomly divided into normal control group,model group,positive group,Lidan Qingyi Jiedu Decoction low-dose and high-dose group with each 10 rats.Bile pancreatic duct ligation was used for modeling.After the model was successfully established,the normal control group and the model group were given intragastric saline,the positive group was given Octreotide(50μg/kg),the low-dose group was given 7.2 g/kg of Lidan Qingyi Jiedu Decoction,the high-dose group was given 21.6 g/kg of Lidan Qingyi Jiedu Decoction,once per day for 7 consecutive days.The pathology and scoring of pancreas tissue in each group rats were compared;tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-10(IL-10)and interleukin-1β(IL-1β)inflammatory factor levels in each group were detected;the intestinal barrier function indicators such as endotoxin(ET),D-lactic acid(DLA),diamine oxidase(DAO)content and amylase(AMY)were determined;Real-time PCR was used to detect the expressions of TRPV1 and CGRP mRNA in rat pancreas.Results:In the model group,the pancreatic tissue scores were significantly increased,serum TNF-α,IL-6,IL-1βlevels,intestinal barrier function indexes,expressions of TRPV1 and CGRP in pancreatic tissue were significantly increased,and IL-10 was significantly reduced(P<0.05);after intervention,the pancreatic tissue scores of the positive group,low-dose group and high-dose group were significantly reduced,TNF-α,IL-6 and IL-1βlevels,intestinal barrier function indexes,TRPV1 and CGRP expressions in pancreas tissue were significantly reduced,and IL-10 levels was significantly increased(P<0.05),there was no significant difference between the positive group and the low-dose group(P>0.05),and there was significant difference between the p
