摘　　要：目的:目的:探讨儿童伸直型肱骨髁上骨折不同手术时机对治疗及预后的影响。方法:回顾性分析我院2018年1月至2020年12月收治的376例儿童肱骨髁上骨折临床资料,按受伤至手术开始时间分为早期组(≤24 h)、延期组(> 24 h)。比较两组的手术时间、术后并发症、骨折愈合情况及末次随访肘关节功能。结果:早期组手术耗时短于延期组(P <0.01);早期组克氏针撬拨复位比例、石膏松动发生率均低于延期组(P <0.01)。两组术后均未出现神经血管损伤、肘内外翻等相关并发症。早期组关节优良率高于延期组(79.82%vs 67.94%,P <0.05)。结论:早期行闭合复位克氏针固定治疗对儿童伸直型肱骨髁上骨折具有显著疗效,可缩短手术耗时,提高关节功能优良率。Objective:To investigate the effect of different operation timing on the treatment and prognosis of children with extended supracondylar humerus fractures.Methods:The clinical data of 376 children with supracondylar humerus fractures treated in our hospital from January 2018 to December 2020 were retrospectively analyzed.According to the time from injury to the start of surgery,they were divided into early group(≤24 hours) and delayed group(> 24 hours).The operation time,postoperative complications,fracture healing and elbow function at the last follow-up were compared between the two groups.Results:The operation time of the early group was significantly shorter than that of the delayed group(P < 0.01);the proportion of Kirschner wire prying reset and the incidence of plaster loosening in the early group were significantly lower than those in the delayed group(P < 0.01).No complications such as neurovascular injury and cubitus varus or valgus occurred in both groups.The excellent and good joint rate of the early group was higher than that of the delayed group(79.82% vs 67.94%,P < 0.05).Conclusion:Early closed reduction and Kirschner wire fixation has a significant effect on extended supracondylar humerus fractures in children,which can shorten the operation time and improve the excellent and good rate of joint function.
