补气温阳通便散治疗老年陈旧性胸腰椎骨折患者椎体成形术术后便秘临床研究

摘　　要：目的:探讨补气温阳通便散治疗老年陈旧性胸腰椎骨折患者椎体成形术(PVP)术后便秘的临床疗效。方法:选取120例老年陈旧性胸腰椎骨折PVP术后便秘患者作为研究对象,随机分为两组。其中常规治疗组59例采用枸橼酸莫沙必利胶囊治疗,通便散组61例加以补气温阳通便散治疗,比较通便散组与常规治疗组中医疗效、中医症候评分、便秘症状评分、血清指标[胃动素(MOT)、活性肠肽(VIP)与生长抑素(SS)]水平变化、不良反应发生情况。结果:治疗后,通便散组中医疗效总有效率为98.36%,明显高于常规治疗组的86.44%(P<0.05);两组中医症候中大便干结、腹满胀痛、口干口臭、纳食减少评分均明显下降(P<0.05),且通便散组治疗后上述评分明显低于常规治疗组(P<0.05);两组便秘症状评分中排便计时、排便频率、排便困难、排便性状分值均明显下降(P<0.05),且通便散组治疗后上述评分明显低于常规治疗组(P<0.05);两组MOT、VIP水平明显上升(P<0.05),SS水平明显下降(P<0.05),且通便散组上述指标与常规治疗组差异明显(P<0.05);两组均未见明显不良反应。结论:补气温阳通便散治疗老年陈旧性胸腰椎骨折患者PVP术后便秘疗效良好,可有效改善患者便秘症状,提高生活质量。Objective:To investigate the clinical efficacy of Buqi Wenyang Tongbian powder in the treatment of constipation after percutaneous vertebroplasty(PVP)in elderly patients with obsolete thoracolumbar vertebral fractures.Methods:A total of 120 elderly patients with constipation after PVP for obsolete thoracolumbar vertebral fractures were selected as the research subjects,and were randomly divided into two groups.Among them,59 cases in the conventional treatment group were treated with mosapride citrate capsules,and 61 cases in the Tongbian powder group were additionally given traditional Chinese medicine Buqi Wenyang Tongbian powder.The efficacy of traditional Chinese medicine,scores of traditional Chinese medicine syndromes,scores of constipation symptoms,levels of serum indicators[motilin(MOT),vasoactive intestinal peptide(VIP),somatostatin(SS)]and occurrence of adverse reactions were compared between Tongbian powder group and conventional treatment group.Results:After treatment,the total effective rate of traditional Chinese medicine efficacy of 98.36%in Tongbian powder group was significantly higher than 86.44%in conventional treatment group(P<0.05).The scores of traditional Chinese medicine syndromes such as dry stool,abdominal fullness and distending pain,dry mouth and bad breath and reduced appetite were significantly decreased in Tongbian powder group and conventional treatment group(P<0.05),and the above scores of Tongbian powder group after treatment were significantly lower than those of control group(P<0.05).The scores of constipation symptoms of defecation time,defecation frequency,defecation difficulty and defecation traits in the Tongbian powder group and conventional treatment group were significantly reduced(P<0.05),and the above scores after treatment were significantly lower in Tongbian powder group than those in control group(P<0.05).The levels of MOT and VIP were increased significantly after treatment(P<0.05),while the level of SS was decreased significantly(P<0.05),and there were significant
