逍遥散加减治疗良性阵发性位置性眩晕手法复位后遗留头昏疗效观察

摘　　要：目的:探究逍遥散加减治疗良性阵发性位置性眩晕(BPPV)手法复位后遗留头昏疗效。方法:回顾性分析BPPV经手法复位后遗留头昏的126例患者,将采用前庭康复训练的患者纳入对照组(n=63),采用逍遥散加减配合前庭康复训练治疗的患者纳入试验组(n=63),比较两组患者疗效、中医症候积分以及头昏严重程度。结果:试验组治疗总有效率高于对照组(P<0.05)。治疗前,两组中医症候积分、眩晕障碍量表(DHI)评分、头晕程度、视觉模拟量表(VAS)评分比较差异无统计学意义(均P>0.05)。两组治疗后各项中医症候积分均下降,且试验组低于对照组(均P<0.05)。两组治疗后DHI评分下降,中度及重度头昏率降低,试验组DHI评分低于对照组,中度及重度头昏率低于对照组(均P<0.05)。两组治疗后VAS评分下降(均P<0.05),组间VAS评分比较差异无统计学意义(P>0.05)。结论:针对BPPV经手法复位后遗留头昏的患者采用逍遥散加减治疗可改善临床症状,降低患者头昏严重程度。Objective:To explore the curative effect of modified Xiaoyao powder in the treatment of residual dizziness symptoms after manual reduction for benign paroxysmal positional vertigo(BPPV).Methods:126 patients who had residual dizziness symptoms after manual reduction for BPPV were retrospectively analyzed.Patients who received vestibular rehabilitation training were included in the control group(n=63),while patients who received treatment with modified Xiaoyao powder and vestibular rehabilitation training were included in the experimental group(n=63).The curative effect,TCM syndrome scores and the severity of dizziness were compared between the two groups.Results:The total treatment response rate in the experimental group was higher than that in the control group(P<0.05).TCM syndrome scores,dizziness handicap inventory(DHI)scores,dizziness degree,and visual analogue scale(VAS)scores showed no significant difference between the two groups before treatment(all P>0.05).After treatment,TCM syndrome scores,DHI scores,and the rate of moderate and severe dizziness decreased,which were lower in the experimental group than those in the control group(all P<0.05).The VAS scores decreased after treatment(all P<0.05),without no significant difference between the groups(P>0.05).Conclusion:Treatment with modified Xiaoyao powder can relieve clinical symptoms and alleviate dizziness in patients with BPPV after manual reduction.
