右美托咪定复合丙泊酚在无痛肠镜检查中的麻醉效果

摘　　要：目的:探究右美托咪定复合丙泊酚在无痛肠镜检查中的麻醉效果,为临床无痛肠镜检查提供一种安全、有效的麻醉方案。方法:选取拟行无痛肠镜诊治的100例患者为研究对象。随机分为单药麻醉组及二药联合组各50例。单药麻醉组开展单纯丙泊酚麻醉,二药联合组则开展右美托咪定及丙泊酚综合麻醉。分析两组麻醉效果优良率、麻醉相关指标水平、麻醉前后认知功能变化情况、不良反应发生情况。结果:二药联合组麻醉效果优良率高于单药麻醉组;二药联合组苏醒时间以及定向力恢复时间均低于单药麻醉组,差异均有统计学意义(均P<0.05);二药联合组麻醉后15 min、麻醉后30 min的简易智能状态检查量表(MMSE)评分均高于单药麻醉组,差异均有统计学意义(均P<0.05)。两组在恶心呕吐、头晕以及低氧血症发生率方面比较差异均无统计学意义(均P>0.05)。结论:右美托咪定复合丙泊酚综合麻醉对无痛肠镜患者麻醉效果较佳,对认知功能的影响程度较轻,且不会增加不良反应发生风险。Objective To study the anesthetic effect of dexmedetomidine combined with propofol in painless enteroscopy,in order to provide a safe and effective anesthesia scheme for clinical painless enteroscopy.Method 100 patients who planned to undergo painless enteroscopy were selected as the research object.They were randomly divided into two drug combination group and single drug anesthesia group,with 50 cases in each group.The single drug anesthesia group was given simple propofol anesthesia,while the two drug combination group was given dexmedetomidine and propofol combined anesthesia.The differences in the excellent and good rate of anesthesia effect,the level of anesthesia related indicators,the changes of cognitive function before and after anesthesia,and the occurrence of adverse reactions between the two groups were analyzed.Results The excellent and good rate of anesthesia effect in the two drug combination group was higher than that in the single drug anesthesia group(P<0.05).The recovery time of awakening and directional force in the two drug combination group were were lower than those in the single drug anesthesia group(all P<0.05).The MMSE scores of the two drug combined group at 15 min and 30 min after anesthesia were higher than those of the single drug anesthesia group(all P<0.05).There was no significant difference in the incidence of nausea,vomiting,dizziness and hypoxemia between the two groups(all P>0.05).Conclusion Dexmedetomidine combined with propofol comprehensive anesthesia has better anesthetic effect on patients with painless enteroscopy,has less impact on cognitive function,and does not increase the risk of adverse reactions.
