摘  要:目的 分析无创正压通气在重度急性左心衰竭合并Ⅱ型呼吸衰竭抢救中的临床应用效果。方法 48例重度急性左心衰竭合并Ⅱ型呼吸衰竭患者,通过随机数字表法分为对照组和实验组,各24例。对照组给予鼻导管供氧治疗,实验组给予无创正压通气治疗。比较两组患者治疗前和治疗1 d后的血压[收缩压(SBP)、舒张压(DBP)]、血浆N-末端脑钠肽前体(NT-proBNP)、动脉血气指标[动脉血二氧化碳分压(PaCO_(2))、氧合指数(PaO_(2)/FiO_(2))]水平。结果 治疗1 d后,实验组的SBP(125.03±12.28)mm Hg(1 mm Hg=0.133 kPa)、DBP(78.17±7.04)mm Hg均低于对照组的(134.68±12.05)、(83.15±9.16)mm Hg,差异具有统计学意义(P<0.05)。治疗1 d后,实验组的血浆NT-proBNP(533.69±100.04)pg/ml低于对照组的(733.25±108.42)pg/ml,差异具有统计学意义(P<0.05)。治疗1 d后,实验组的PaCO_(2)(51.39±6.43)mm Hg低于对照组的(60.22±5.97)mm Hg,PaO_(2)/FiO_(2)(239.88±20.05)mm Hg高于对照组的(217.64±19.85)mm Hg,差异具有统计学意义(P<0.05)。结论 在重度急性左心衰竭合并Ⅱ型呼吸衰竭患者的临床抢救中,对其进行无创正压通气治疗不仅可以有效改善患者的氧合指数和二氧化碳潴留,还能明显改善患者的心功能及血压,具有良好的临床应用效果。Objective To analyze the clinical effect of non-invasive positive pressure ventilation in rescuing severe acute left heart failure complicated with typeⅡrespiratory failure.Methods A total of 48 patients with severe acute left heart failure and typeⅡrespiratory failure were divided into the control group and the experimental group by random numerical table,with 24 cases in each group.The control group was treated with nasal cannula oxygen therapy,while the experimental group was treated with non-invasive positive pressure ventilation.Both groups were compared in terms of blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)],plasma N-terminal pro-brain natriuretic peptide(NT-proBNP),arterial blood gas indexes[arterial partial pressure of carbon dioxide(PaCO_(2)),oxygenation index(PaO_(2)/FiO_(2))]levels before treatment and after 1 d of treatment.Results After 1 d of treatment,the SBP(125.03±12.28)mm Hg(1 mm Hg=0.133 kPa)and DBP(78.17±7.04)mm Hg of the experimental group were lower than(134.68±12.05)and(83.15±9.16)mm Hg of the control group,and the differences were all statistically significant(P<0.05).After 1 d of treatment,the plasma NT-proBNP(533.69±100.04)pg/ml of the experimental group was lower than(733.25±108.42)pg/ml of the control group,and the difference was statistically significant(P<0.05).After 1 d of treatment,the PaCO_(2)(51.39±6.43)mm Hg of the experimental group was lower than(60.22±5.97)mm Hg of the control group;the PaO_(2)/FiO_(2)(239.88±20.05)mm Hg of the experimental group was higher than(217.64±19.85)mm Hg of the control group;the differences were all statistically significant(P<0.05).Conclusion For patients with severe acute left heart failure complicated with typeⅡrespiratory failure,non-invasive positive pressure ventilation can not only effectively improve the oxygenation index and carbon dioxide retention of the patients,but also significantly improve the cardiac function and blood pressure of patients,which contains a good clinical application effec
