摘 要:目的 探究经鼻间歇正压通气(NIPPV)治疗早产儿呼吸窘迫综合征的临床效果。方法 86例呼吸窘迫综合征早产儿,依据随机数字表法分为对照组与观察组,各43例。对照组接受经鼻持续气道正压通气(NCPAP)、观察组接受NIPPV治疗。比较两组不同治疗时间的血氧水平、并发症发生情况、预后情况。结果 治疗12 h、治疗24 h、治疗后,观察组动脉血氧分压(PaO_(2))、氧合指数(OI)分别为(50.39±5.88)、(58.70±6.29)、(59.01±5.47)mm Hg(1 mm Hg=0.133 kPa)与(46.71±4.79)、(54.15±5.35)、(56.67±5.30)mm Hg,高于对照组的(160.43±10.37)、(218.84±27.45)、(267.13±24.22)mm Hg与(151.74±13.42)、(197.45±24.37)、(254.46±19.47)mm Hg,差异有统计学意义(P>0.05)。观察组呼吸机相关性肺炎发生率为2.33%,低于对照组的13.95%,差异有统计学意义(P<0.05);两组脑室出血、动脉导管未闭、支气管肺发育不良、气胸发生率比较差异无统计学意义(P>0.05)。观察组机械通气时间(51.44±9.67)h、住院时间(32.45±3.44)d短于对照组的(57.53±7.25)h、(35.53±5.97)d,肺表面活性物质使用率23.26%、呼吸暂停发生率25.58%低于对照组的44.19%、55.81%,差异均有统计学意义(P<0.05)。结论 早产儿呼吸窘迫综合征应用NIPPV治疗,可改善患儿血氧状态,降低呼吸机相关性肺炎发生率,改善预后,效果理想。Objective To investigate the clinical effect of nasal intermittent positive pressure ventilation(NIPPV)in the treatment of respiratory distress syndrome in premature infants.Methods A total of 86 cases of premature infants with respiratory distress syndrome were divided into control group and observation group according to random numerical table,with 43 cases in each group.The control group received nasal continuous positive airway pressure(NCPAP),and the observation group received NIPPV treatment.The blood oxygen levels at different treatment time,occurrence of complications and prognosis of the two groups were compared.Results After 12 h of treatment,24 h of treatment and after treatment,the arterial partial pressure of oxygen(PaO_(2))and oxygenation index(OI)in the observation group were(50.39±5.88),(58.70±6.29),(59.01±5.47)mm Hg(1 mm Hg=0.133 kPa),and(46.71±4.79),(54.15±5.35),(56.67±5.30)mm Hg,which were higher than(160.43±10.37),(218.84±27.45),(267.13±24.22)mm Hg and(151.74±13.42),(197.45±24.37),(254.46±19.47)mm Hg in the control group,and the differences were all statistically significant(P<0.05).The incidence of ventilator-related pneumonia in the observation group was 2.33%,which was lower than 13.95%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of intraventricular hemorrhage,patent ductus arteriosus,bronchopulmonary dysplasia and pneumothorax between the two groups(P>0.05).The duration of mechanical ventilation(51.44±9.67)h and hospital stay(32.45±3.44)d in the observation group were shorter than(57.53±7.25)h and(35.53±5.97)d in the control group;the utilization rate of pulmonary surfactant 23.26%,and incidence of apnea 25.58%in the observation group were lower than 44.19%and 55.81%in the control group;the differences were all statistically significant(P<0.05).Conclusion NIPPV has ideal effect on premature infants,and can improve the blood oxygen status,reduce the incidence of ventilator-ass
