全凭静脉麻醉对老年患者术后认知功能的影响

摘　　要：目的 探讨全凭静脉麻醉对60岁以上患者术后认知功能的影响。方法 70例老年手术患者,按照患者临床麻醉方式不同分为实验组和对照组,每组35例。对照组实施常规麻醉,实验组患者实施全凭静脉麻醉。比较两组患者麻醉前后抑郁自评量表(SDS)、焦虑自评量表(SAS)、认知功能评分。结果 麻醉后,两组患者SAS、SDS评分均低于麻醉前,实验组SAS评分(29.36±3.21)分、SDS评分(21.58±2.33)分均低于对照组的(46.32±3.21)、(39.63±3.21)分,差异均具有统计学意义(P<0.05)。麻醉后,两组患者认知功能评分均高于麻醉前,且实验组认知功能评分(76.36±2.52)分高于对照组的(64.25±2.42)分,差异具有统计学意义(P<0.05)。结论 在60岁以上的老年患者手术麻醉中,实施全凭静脉麻醉的价值优于常规麻醉,对患者术后认知功能控制和管理有一定帮助。Objective To discuss the effects of total intravenous anesthesia on postoperative cognitive function in patients over 60 years old.Methods A total of 70 cases of elderly surgical patients were divided into experimental group and control group according to different clinical anesthesia methods,with 35 cases in each group.The control group received conventional anesthesia,while the experimental group received total intravenous anesthesia.The self-rating depression scale(SDS),self-rating anxiety scale(SAS)and cognitive function scores before and after anesthesia were compared between the two groups.Results After anesthesia,the SAS and SDS scores of the two groups were lower than those of this group before anesthesia,and the SAS score(29.36±3.21)points and SDS score(21.58±2.33)points of the experimental group were lower than(46.32±3.21)and(39.63±3.21)points of the control group,and the differences were statistically significant(P<0.05).After anesthesia,the cognitive function scores of the two groups were higher than those of this group before anesthesia,and the cognitive function score(76.36±2.52)points of the experimental group was higher than(64.25±2.42)points of the control group,and the difference was statistically significant(P<0.05).Conclusion In surgical anesthesia for elderly patients over 60 years old,the value of total intravenous anesthesia is better than that of conventional anesthesia,which is helpful for the control and management of postoperative cognitive function of patients.
