摘 要:目的:探究新型可吸引经尿道输尿管一体硬镜关于模拟肾盂压与灌注流量的体外实验效果。方法:仿真模拟经尿道输尿管硬镜碎石术情景,使用新型输尿管硬镜,记录不同场景下仿真肾盂集合系统的压强、灌注流量。结果:悬挂灌注液的相对高度<54cm、灌注和吸引双通道内有液体流动时,模拟肾盂压远低于30mmHg安全阈值上限;中心负压吸引是调节灌注流量的主要决定因素,术中灌注流量:33.9mL/min以上;不置入光纤时,灌注液的流量随连接的中心负压值增大而明显升高,呈正相关关系;置入光纤后,灌注液的流量随连接的中心负压绝对值增大而明显升高,呈正相关关系;随置入光纤外径增大而明显降低,呈负相关关系。结论:新型可吸引经尿道输尿管一体硬镜能确保安全的模拟肾盂压并达到理想的术中灌注流量,具有较好的临床应用前景。Objective:To investigate the in vitro experimental effect of a new rigid ureteroscope capable of attracting transurethral ureter on the simulation of renal pelvis pressure and perfusion flow.Methods:The situation of transurethral ureteroscopic lithotripsy was simulated,and the pressure and perfusion flow of the simulated renal pelvis collecting system were recorded under different scenarios using a new type of ureteroscopic lithotripsy.Results:When the relative height of hanging perfused fluid was less than 54cm and there was fluid flow in both perfusion and suction channels,the simulated renal pelvis pressure was well below the upper limit of the safety threshold of 30mmHg.Central negative pressure suction was the main determinant of perfusion flow.Intraoperative perfusion flow was more than 33.9mL/min.When the optical fiber was not inserted,the flow rate of the perfusate increased significantly with the increase of the negative pressure value of the center of the connection,and there was a positive correlation.The flow rate of perfusate increased significantly with the increase of the absolute value of the center negative pressure,and there was a positive correlation.With the increase of the outer diameter of the embedded fiber,it decreased obviously,showing a negative correlation.Conclusion:The new rigid ureteroscope can safely simulate the renal pelvis pressure and achieve the ideal intraoperative perfusion flow.It has a good clinical application prospect.
