改良微创前路与直接前路全髋关节置换的效果比较

摘　　要：目的:总结改良微创前路全髋关节置换术围手术期的临床疗效。方法:分析我院开展改良微创前路全髋关节置换术47例患者的临床资料,设为研究组,同时选取同期直接外侧入路全髋关节置换术患者47例,设为对照组。比较两组患者术中出血、切口长度、下床时间、住院天数及Harris评分与Barthel指数的差异。结果:术后随访14~26个月,两组患者术中出血量、切口长度、下床时间、住院天数比较差异有统计学意义(P<0.05);两组髋关节Harris评分随时间变化趋势差异有统计学意义(P<0.05);两组术后日常生活能力评定Barthel指数随时间变化趋势差异有统计学意义(P<0.05)。结论:前侧经肌肉间隙入路微创(DAMIS)全髋关节置换术相比较标准外侧入路全髋关节置换术,具有微创、出血少、术后恢复更快的优势,有利于患者快速康复。Objective:To summarisetheclinical effectof rapid rehabilitation in perioperative period of modified minimally invasive anterior total hip arthroplasty.Methods:The clinical data of 47 patients with total hip replacementbyimproved minimally invasive anterior approach wasanalysed,andthey were set as study group;At the same time,47 patients with total hip replacementbythe standard of the lateral approach was set as control group.The difference of two groupswas compared,including operation time,intraoperative bleeding,incision length,out-of-bed time,hospitalization days,Harris score and Barthel index.Results:Postoperative follow-up for 14 to 26 months,the operation time,intraoperative blood loss,length of incision,out-of-bed time,hospitalization days of the two groups were statistically significant(P<0.05).There was a statistically significant difference between the two groups in hip joint Harris scores over time(P<0.05);There was a statistically significant difference between the two groups in postoperative activities of daily living assessment Barthel index over time(P<0.05).Conclusion:Compared with the standard lateral approach total hip arthroplasty,the anterior transmuscular approach minimally invasive(DAMIS)has the advantages of minimally invasive,less bleeding and faster postoperative recovery,which is conducive to the rapid recovery of patients.
