超声造影与弹性成像对颈部异常淋巴结诊断价值的评估

摘　　要：目的:分析并评价超声造影(contrast-enhanced ultrasound,CEUS)与超声弹性成像(ultrasonic elastography,UE)在颈部异常淋巴结诊断中的价值。方法:选取我院2018年9月—2021年3月行CEUS和UE检查的55例患者共69个颈部异常淋巴结,结合每个淋巴结的声像图特点予以赋值评分,与病理结果进行对比,构建ROC曲线,比较CEUS、UE单独检查及联合检查在颈部异常淋巴结诊断中的价值。结果:69个颈部异常淋巴结中,病理学诊断恶性58个,良性11个,单独采用CEUS与UE检查分析淋巴结的ROC曲线下面积(AUC)、特异度、敏感度、准确性分别为0.857、72.73%、82.76%、81.16%与0.803、63.64%、70.70%、69.57%。联合组ROC曲线下面积(AUC)为0.850,其特异度、敏感度、准确性分别为45.45%、94.83%、86.96%,联合组敏感度与准确性明显优于UE组,差异有统计学意义(P<0.05),联合组与CEUS组差异无统计学意义(P>0.05)。UE组、CEUS组及联合组特异度比较,差异无统计学意义(P>0.05)。结论:相对于UE单独检查,CEUS检查与联合检查更能准确诊断颈部异常淋巴结。Objective:To analysis and evaluate the value of contrast-enhanced ultrasound(CEUS)and ultrasonic elastography(UE)in diagnosing cervical benign and malignant lymph nodes.Methods:From September 2018 to March 2021,a total of 69 abnormal cervical swollen lymph nodes in 55 patients who underwent CEUS and UE in Yijishan Hospital of Wannan Medical College were included.The scores were evaluated according to the sonographic characteristics of each lymph node.The diagnostic results were compared with biopsy or operative pathology.ROC curve was used to analyze the value of CEUS and UE examination alone and combined examination in the diagnosis of benign and malignant cervical lymph nodes.Results:Among the 69 lesions,pathological diagnosis showed malignant lesions of 58,and benign lesions of 11.CEUS and UE examinations alone were used in the area under the ROC curve(AUC),the specificity,sensitivity,and accuracy were 0.857,72.73%,82.76%,81.16%,and 0.803,63.64%,70.70%,69.57%,respectively.The AUC of combined group was 0.850,and the specificity,sensitivity,and accuracy were 45.45%,94.83%,and 86.96%,respectively,the sensitivity and accuracy were higher than UE scoring,with statistical significance(P<0.05),but there was no significant difference between the combined group and CEUS group(P>0.05).The specificity was no significant difference between the combined group and independent groups(P>0.05).Conclusion:Compared withUE group,CEUSgroupandcombinedgroupcansignificantlyimprovetheaccuracyinthediagnosisofabnormal cervical swollen lymph nodes.
