摘　　要：体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)作为一种体外循环支持技术,通过将患者静脉血引出在体外进行气体交换后回输到患者体内,代替了患者部分心肺功能,为呼吸衰竭及循环衰竭患者原发病的治疗及心肺功能的恢复争取了时间。常用的ECMO模式有静脉-静脉体外膜肺氧合(Veno-venous extracorporeal membrane oxygenation,VV ECMO)和静脉-动脉体外膜肺氧合(Veno-arterial extracorporeal membrane oxygenation,VA ECMO)。床旁超声作为一种实时、动态、无辐射、可移动、可重复检查手段,可用于危重症患者的检查。本文就床旁超声在ECMO模式选择中的作用、在ECMO置管时的指导、ECMO期间的监测及指导ECMO撤机等各阶段的应用进行综述。As a cardiopulmonary support technique,extracorporeal membrane oxygenation(ECMO)replaces part of the cardiopulmonary function by extracting the patient's venous blood for gas exchange in vitro and then injecting it back into the patient's body,which strivesfor time for the treatment of primary disease and recovery of cardiopulmonary function in patients with respiratory and circulatory failure.The commonly used ECMO modes are veno-venous extracorporeal membrane oxygenation(VV ECMO)and veno-arterial extracorporeal membrane oxygenation(VA ECMO).As a real-time,dynamic,radiation-free,mobile and repeatable examination method,bedside ultrasound can be used for the examination of critically ill patients.In this paper,the role of bedside ultrasound in ECMO mode selection,guidance during ECMO catheterization,monitoring during ECMO and application of ECMO offline are reviewed.
