加用附辛汤治疗心阳不振型病态窦房结综合征疗效观察

摘　　要：目的:观察加用附辛汤治疗老年心阳不振型病态窦房结综合征的疗效。方法:选取老年心阳不振型病态窦房结综合征患者60例,随机分为对照组与治疗组各30例。两组均采用西医常规治疗,对照组加予心宝丸360 mg(6丸)口服,3次/天;治疗组加用附辛汤(制附子15 g,细辛5 g,黄芪30 g,川芎10 g,桂枝15 g,薤白15 g,瓜蒌皮10 g),每日1剂,分早晚服用。两组疗程均为60 d。比较两组治疗前后主要中医证候积分、24 h总窦性心率、平均窦性心率、血D-二聚体(D-dimer)及氨基末端脑钠肽前体(NT-pro BNP)水平,统计两组疗效。结果:治疗组总有效率为93.33%,对照组为76.67%,两组疗效差异有统计学意义(P<0.05);两组中医证候积分均较治疗前降低,且治疗组更优,差异具有统计学意义(P<0.05)。治疗后两组患者的24 h总窦性心率及平均窦性心率均高于治疗前,且治疗组高于对照组,差异具有统计学意义(P<0.05)。治疗后两组患者血D-dimer及NT-pro BNP水平均下降,且治疗组较对照组更明显,差异具有统计学意义(P<0.05)。结论:在西医常规治疗的基础上加用附辛汤可提高心阳不振型病态窦房结综合征的疗效,改善心悸、胸闷、乏力、畏寒等症状,提高窦性心率,有效改善循环,增强心功能。Objective:o observe the curative effects of additional Fuxin decoction in treating elderly sick sinus syndrome(SSS)with heart-yang deficiency.Methods:A total of 60 elderly patients diagnosed as SSS with heart-yang deficiency were randomly and divided into two groups,with 30 cases in both control and treatment group.Both groups were treated with Western conventional medicine.At the same time,the control group orally administrated additional 360 mg Xinbao pill(6 pills)3 times a day while the treatment group received additional Fuxin decoction[15 g Zhifuzi(prepared aconite root;Radix Aconiti Lateralis Praeparata),5 g Xixin(asarum;Radix et Rhizoma Asari),30 g Huangqi(astragalus root;Radix Astragali),10 g Chuanxiong(cnidium;Rhizoma Chuanxiong),15 g Guizhi(cassia twig;Ramulus Cinnamomi),15 g Xiebai(long stamen onion bulb;Bulbus Allii Macrostemi),10 g Gualoupi(trichosanthes peel;Pericarpium Trichosanthis)],1 dose a day for morning and evening.The course of treatment for both groups was 60 days.Traditional Chinese Medicine(TCM)Syndrome Score,24-hour total sinus heart rate,mean sinus heart rate,D-dimer and NT-pro BNP levels were compared between the two groups before and after treatment to measure their curative effects.Results:The total effective rate was 93.33%in the treatment group and 76.67%in the control group,and the difference between the two groups was statistically significant(P<0.05);the TCM syndrome score in both groups were lower after treatment,even lower in the treatment group,and the difference was statistically significant(P<0.05).The 24-hour total sinus heart rate and mean sinus heart rate of both groups were higher than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the D-dimer and NT-pro BNP levels decreased in both groups,and the difference was more obvious in the treatment group than in the control group(P<0.05).Conclusion:The addition of Fuxin decoction to the Western conventional treatment can improve the curative effects in treating the SSS with hear
