系统免疫炎症指数对突发性聋患者预后的影响

摘　　要：目的探讨中性粒细胞与淋巴细胞之比(NLR)、血小板与淋巴细胞之比(PLR)、系统免疫炎症指数(systemic immune inflammation index,SII)对突发性聋预后的影响。方法100例单侧突发性聋确诊患者根据治疗效果分为有效组59例和无效组41例,采用单因素和多因素Logistic分析比较两组治疗前的NLR、PLR、SII;采用ROC曲线法分析白细胞计数、中性粒细胞计数、NLR和SII对突发性聋患者预后的预测价值,并确定其最佳截断值。结果单因素分析结果显示突发性聋预后与听阈曲线分型、听力损失程度、白细胞计数、中性粒细胞计数、NLR、SII有关(P<0.05),而与初诊时间、住院天数、性别、年龄、单核细胞计数、淋巴细胞计数、血小板无关(P>0.05)。采用二元Logistic回归分析进行影响因素分析,结果显示白细胞计数、中性粒细胞计数、NLR、SII为突发性聋预后的影响因素。ROC曲线分析结果显示,白细胞计数、中性粒细胞计数、NLR、SII预测突发性聋患者预后不良的AUC面积分别为0.850、0.875、0.792、0.800。结论NLR、SII对突发性聋患者的预后具有一定的预测作用。Objective To investigate the correlation of neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),and systemic immune inflammation index(SII)and the prognosis in patients with sudden deafness.Methods A total of 100 confirmed cases of sudden deafness were included in this study.According to the treatment effect,they were divided into effective and ineffective groups,of which 59 cases were in the effective group and 41 cases were in the ineffective group.Single factor analysis and multivariate logistic regression analysis were used to compared the indicators of two groups before treatment,including NLR,PLR,and SII.And the ROC curve was used to examine the predictive power of white blood cell,neutrophil,NLR,SII on the prognosis of patients with sudden deafness,and determine their optimal cut-off values.Results The univariate analysis showed that hearing classification,degree of hearing loss,white blood cell,neutrophil,NLR,SII were correlated with prognosis of sudden hearing loss(P<0.05),but not the time of initial diagnosis,length of hospitalization,gender,age,monocyte,lymphocyte,platelet(P>0.05).Binary logistic regression analysis was used to analyze the influencing factors.The results showed that white blood cell,neutrophil,NLR,and SII were independent risk factors for the prognosis of sudden deafness.The result of ROC curve analysis showed that the AUC areas of white blood cell,neutrophil,NLR,and SII for predicting poor prognosis of patients with sudden deafness were 0.850,0.875,0.792,and 0.800,respectively.Conclusion NLR and SII have a certain predictive effect on the prognosis of patients with sudden deafness.
