彩超引导下经皮腔内高压球囊扩张血管成形术治疗自体动静脉内瘘狭窄的效果分析

摘　　要：目的 探讨彩超引导下经皮腔内高压球囊扩张血管成形术治疗自体动静脉内瘘狭窄的安全性及效果。方法 18例动静脉内瘘狭窄患者,均应用彩超引导下经皮腔内高压球囊扩张血管成形术治疗,观察手术情况、术后随访情况,比较手术前后血管情况。结果 18例患者手术均成功,手术成功率为100.0%。术后所有患者均未见血管出血、破裂及血栓等并发症发生。术后,狭窄处内径(4.37±0.58)mm、透析血流量(251.37±18.29)ml/min大于术前的(2.01±0.56)mm、(182.24±15.19)ml/min,狭窄处峰值流速(210.67±30.45)cm/s慢于术前的(490.50±44.31)cm/s,差异有统计学意义(P<0.05)。术后随访12个月,患者半年内再干预率为5.6%(1/18), 1年内再干预率为22.2%(4/18), 1年通畅率为72.2%(13/18)。结论 彩超引导下经皮腔内高压球囊扩张血管成形术治疗自体动静脉内瘘狭窄简便易行,安全性高,疗效确切,具有良好的应用价值。Objective To discuss the safety and effect of percutaneous transluminal angioplasty guided by color Doppler ultrasound in the treatment of autologous arteriovenous fistula stenosis.Methods 18 patients with autologous arteriovenous fistula stenosis were treated with percutaneous transluminal angioplasty guided by color Doppler ultrasound.The surgery and postoperative follow-up were observed,and the vascular conditions before and after surgery were compared.Results The surgery was successful in all 18 patients,with a success rate of 100.0%.No postoperative complications such as vascular bleeding,rupture and thrombosis were observed in all patients.After surgery,the inner diameter of the stenosis was(4.37±0.58)mm and the dialysis blood flow was(251.37±18.29)ml/min,which were greater than(2.01±0.56)mm and(182.24±15.19)ml/min before surgery;the peak flow velocity at the stenosis(210.67±30.45)cm/s was slower than(490.50±44.31)cm/s before surgery;the differences were all statistically significant(P<0.05).After 12 months of follow-up,the re-intervention rate within 6 months was 5.6%(1/18),the re-intervention rate within 1 year was 22.2%(4/18),and the 1-year patency rate was 72.2%(13/18).Conclusion Percutaneous transluminal angioplasty guided by color Doppler ultrasound in the treatment of autologous arteriovenous fistula stenosis is simple and easy,with high safety and definite efficacy,and has good application value.
