心脏冠脉CTA后处理在心肌桥诊断中的应用价值

摘　　要：目的 研究心脏冠状动脉(冠脉)CT血管造影(CTA)后处理在心肌桥诊断中的应用价值。方法 回顾性分析220例进行心脏冠脉CTA后处理技术诊断患者的临床资料。所有患者均进行冠脉造影诊断,并将冠脉造影诊断作为金标准,分析心脏冠脉CTA后处理技术在心肌桥诊断中的诊断价值。结果 经冠脉造影检出心肌桥患者35例,经心脏冠脉CTA后处理技术检出32例,对比差异无统计学意义(P>0.05)。以冠脉造影结果为金标准,心脏冠脉CTA后处理技术诊断心肌桥的阳性预测值、阴性预测值、敏感度、特异度、准确度、约登指数、误诊率、漏诊率分别为96.88%、97.87%、88.57%、99.46%、97.73%、0.88、0.54%、11.43%。结论 通过心脏冠脉CTA后处理技术能够实现对心肌桥的诊断,可以为临床的诊断和治疗工作提供科学的参考依据,是值得推荐的辅助诊断手段。Objective To study the practical value of cardiac coronary CT angiography(CTA)postprocessing in the diagnosis of myocardial bridges.Methods The clinical data of 220 patients who underwent the diagnosis of cardiac coronary CTA post-processing were retrospectively analyzed.All patients underwent diagnosis of coronary angiography,and the diagnosis of coronary angiography was used as the gold standard to analyze the diagnostic results and application value of cardiac coronary CTA post-processing in the diagnosis of myocardial bridge.Results There were 35 patients with myocardial bridges confirmed by coronary angiography and 32 patients with myocardial bridges detected by cardiac coronary CTA post-processing,and the difference was not statistically significant(P>0.05).Taking the results of coronary angiography as the gold standard,the positive predictive value,negative predictive value,sensitivity,specificity,accuracy,Jorden index,misdiagnosis rate,and missed diagnosis rate of cardiac coronary CTA post-processing for the diagnosis of myocardial bridge were 96.88%,97.87%,88.57%,99.46%,97.73%,0.88,0.54%,and 11.43%,respectively.Conclusion Cardiac coronary CTA post-processing can achieve the diagnosis of myocardial bridges,which can provide a scientific reference basis for the clinical diagnosis and treatment,and is a recommended auxiliary diagnosis method.
