乳腺癌不同CT征象与雌激素受体、孕激素受体及C-erbB-2表达的关系

摘　　要：目的初探乳腺癌不同CT征象与雌激素受体(ER)、孕激素受体(PR)、C-erbB-2表达水平的关系。方法选取本院2017年9月至2019年9月收治的79例乳腺癌患者的临床资料,均经手术病理或穿刺活检确诊,并在此前行CT检查。观察患者所得CT图像,计算ER、PR、C-erbB-2在乳腺癌患者中表达阳性情况,并分析CT征象与ER、PR、C-erbB-2表达水平的相关性。结果在79例乳腺癌患者中,C-erbB-2表达阳性有32.91%(26/79),ER表达阳性有65.82%(52/79),PR表达阳性有69.62%(55/79),ER、PR均阳性患者有46.82%(37/79)。乳腺癌患者ER、PR阳性表达者,出现毛刺针机率明显高于ER、PR阴性者(P﹤0.05),在肿瘤大小、分叶征、钙化、肿瘤密度、淋巴结转移情况比较无差异(P﹥0.05);C-erbB-2表达阳性者出现肿瘤直径﹤5cm、钙化、淋巴结转移情况明显高于C-erbB-2表达阴性者(P﹤0.05),两者在分叶征、毛刺征、肿瘤密度中比较无差异(P﹥0.05)。结论乳腺癌不同CT征象与ER、PR、C-erbB-2存在相关性,可为乳腺癌患者治疗方案及预后情况评估提供参考。Objective To explore the relationship between different CT signs of breast cancer and the expression levels of estrogen receptor(ER),progesterone receptor(PR)and C-erbB-2.Method The clinical data of 79 patients with breast cancer admitted to our hospital from September 2017 to September 2019were selected and confirmed by surgical pathology or puncture biopsy,and CT examination was performed before that.The CT images of the patients were observed,the positive expression of ER,PR,C-erbB-2 in patients with breast cancer was calculated,and the correlation between CT signs and the expression levels of ER,PR,C-erbB-2 was analyzed.Results In 79 breast cancer patients,32.91%(26/79)patients had positive C-erbB-2 expression,65.82%(52/79)patients had positive ER expression,and 69.62%(55/79)patients had PR positive expression,46.82%(37/79)patients had positive ER and PR.In patients with breast cancer patients and positive ER and PR expressions,the incidence of burr was significantly higher than that of patients with negative ER and PR(P﹤0.05),and there was no difference in tumor size,lobulation sign,calcification,tumor density,and lymph node metastasis(P﹥0.05).In those patients with positive expression of C-erbB-2,the incidence of tumor diameter﹤5cm,calcification,and lymph node metastasis was significantly higher than that of patients with negative expression of C-erbB-2(P﹤0.05).There was no difference between the them in lobulation sign,burr sign and tumor density(P﹥0.05).Conclusion Different CT signs of breast cancer are correlated with ER,PR,C-erbB-2,which can provide a reference for the evaluation of the treatment plan and prognosis of patients with breast cancer.
