摘　　要：目的:了解糖尿病足(DF)患者住院诊疗费用的现状及其影响因素。方法:对医院内分泌科住院诊疗的332例DF患者的电子病历资料进行回顾性分析,了解DF患者住院诊疗费用情况;并应用多元线性回归分析了解DF住院诊疗费用的独立影响因素。结果:332例DF患者平均年龄(63.17±11.875)岁,平均住院时间14(11,18)天,下肢血管病变发病率高达81.02%,截肢率为27.71%,平均住院总费用(32753.67±1387.65)元。住院天数、医保支付方式、疾病诊断相关分组(DRG)付费、年份、既往行血管成形术、Wagner分级、合并有下肢血管病变、高血压病和冠心病、血HDL-C、ALB、肌酐水平均与住院总费用相关(P<0.05);多元线性回归分析显示,住院天数、年份、Wagner分级、血清白蛋白及血管成形术是住院费用主要影响因素(P<0.05)。结论:DF病情复杂,治疗费用高昂,强调早期综合防治、加强病情评估,开展多学科合作,可能是行之有效的途径。Objective:This paper aims to explore the status and influencing factors of hospitalization expenses for patients with diabetic foot(DF).Methods:The electronic medical records of 332 DF patients hospitalized in the Department of Endocrinology of the hospital were retrospectively analyzed to understand the hospitalization expenses of DF patients;Multiple linear regression analysis was used to explore the independent influencing factors of DF hospitalization expenses.Results:The average age of 332 DF patients was(63.17±11.875)years old,the average hospitalization time was 14(11,18)days.The incidence rate of lower limb vascular lesions was 81.02%,the major amputation rate was 27.71%and the average hospitalization total cost was(32753.67±1387.65)yuan.The length of hospital stay,payment method of medical insurance,diagnosis related groups(DRG)payment,particular year,previous angioplasty,Wagner grade,combined with lower extremity vascular disease,hypertension and coronary heart disease,the levels of blood HDL-C,ALB and creatinine were all related to the total cost of hospitalization(P<0.05).Multiple linear regression analysis showed that hospitalization days,particular year,Wagner grade,serum albumin ALB and angioplasty were the main influencing factors of hospitalization expenses(P<0.05).Conclusion:The condition of DF is complex and the treatment cost is high.It may be an effective way to emphasize early comprehensive prevention and treatment,strengthen condition evaluation and carry out multidisciplinary cooperation.
