摘　　要：目的:探讨侧卧位有限切开股骨近端防旋髓内钉(PFNA)内固定自体骨泥植骨治疗股骨粗隆间骨折的临床疗效。方法:对21例采用牵引床仰卧位和侧卧位闭合复位不良的难复性粗隆间骨折患者采用侧卧位有限切开PFNA内固定自体骨泥植骨治疗。结果:手术时间40~150 min,平均(76.5±11.3)min。术中出血60~300 mL,平均(106.3±15.8)mL。术后X线示复位均良好。21例患者随访8~20个月,平均(10.3±1.2)个月,骨折全部愈合。根据髋关节功能Harris评分标准评定疗效,优13例,良7例,可1例。结论:采用侧卧位有限切开PFNA内固定自体骨泥植骨治疗难复性粗隆间骨折,手术安全,复位满意,内固定可靠,术后功能恢复良好。Objective:To investigate the clinical effect of limited incision on the lateral position of proximal femur with anti-rotation intramedullary nail(PFNA)internal fixation and autogenous bone grafting in the treatment of femoral intertrochanteric fractures.Methods:21 cases of refractory intertrochanteric fractures with poor closed reduction in the supine and lateral positions of the traction bed were treated with limited incision in the lateral position and PFNA internal fixation with autogenous bone grafting.Results:The operation time was 40-150 minutes,with an average of(76.5±11.3)minutes;the intraoperative blood loss was 60-300 mL,with an average of(106.3±15.8)mL,and the postoperative X-ray showed good reduction.21 patients were followed up for 8-20 months,with an average follow-up of(10.3±1.2)months,all fractures healed.According to the Harris scoring of hip joint function,13 cases were excellent,7cases were good,and 1 case was medium.Conclusions:The use of lateral decubitus limited incision and PFNA internal fixation for the treatment of refractory intertrochanteric fractures with autogenous bone grafting is recommendable withsafety operation,satisfactory reduction,reliable internal fixation,and good postoperative functional recovery.
