摘 要:目的:对比实时荧光定量P C R法与涂片法检测结核分枝杆菌(MTB)的效果。方法:选取2021年1月—12月滨州市中心医院结核科三病区住院肺结核患者144例,非结核患者41例。所有患者均接受涂片法和实时荧光定量PCR法进行结核分枝杆菌检测,观察两种检测方式的诊断效果。结果:实时荧光定量PCR法阳性率(60.00%)高于涂片检测法(42.16%),差异有统计学意义(P<0.05);实时荧光定量PCR法诊断灵敏度、特异度、准确度及阴性预测值均高于涂片法,差异有统计学意义(P<0.05);两种检测方法诊断阳性预测值比较,差异无统计学差异(P>0.05)。结论:实时荧光定量PCR法优于涂片法,能够提高MTB检测的阳性率和诊断准确率,为患者后续治疗提供有利的诊断依据,值得临床应用。Objective To compare the effect of real-time fluorescent quantitative PCR and smear method in detecting Mycobacterium tuberculosis(MTB).Methods From January to December 2021,144 pulmonary tuberculosis patients and 41 non tuberculosis patients in the third ward of the tuberculosis department of Binzhou Central Hospital were selected.All patients received smear method and real-time fluorescence quantitative PCR method to detect Mycobacterium tuberculosis and observe the diagnostic effect of the two detection methods.Results The positive rate of real-time fluorescent quantitative PCR(60.00%)was significantly higher than that of smear(42.16%)(P<0.05);The sensitivity,specificity,accuracy and negative predictive value of real-time fluorescent quantitative PCR were higher than those of smear(P<0.05);There was no significant difference in the positive predictive value of diagnosis between the two detection methods(P>0.05).Conclusion Real time fluorescent quantitative PCR method is superior to smear method,which can improve the positive rate and diagnostic accuracy of MTB detection and provide favorable diagnostic basis for follow-up treatment of patients,and is worthy of clinical application.
