达格列净治疗射血分数降低心力衰竭的临床效果观察

摘　　要：目的观察达格列净治疗射血分数降低心力衰竭(heart failure with reduced ejection fraction,HFrEF)患者的临床疗效。方法选取2021年2月~12月在宁国市人民医院心内科住院及门诊就诊的射血分数降低的患者120例,根据治疗方案分为两组,剔除失访病例,对照组入组50例,对照组为常规抗心力衰竭治疗方案;观察组入组51列,治疗方案为对照组基础上加用达格列净10 mg每日1次。治疗时间60 d及以上,观察患者治疗前后B型脑钠肽原(proBNP)、左室舒张末径(LDV)、左室射血分数(LVEF)、血浆醛固酮、6 min步行距离等相关指标,并统计心衰再住院、心源性死亡等。结果观察组患者治疗后proBNP、LDV、LVEF、醛固酮、6 min步行距离等相关指标均有明显改善,改善程度较对照组更为显著,差异有统计学意义(P<0.05)。结论达格列净治疗HFrEF疗效显著,相关指标改善更明显,其改善心衰作用可能部分与抑制肾素血管经张素醛固酮系统(RAAS)有关。Objective To observe clinical effect of Dapagliflozin in treating congestive heart failure with reduced ejection fraction.Methods A total of 120 patients admitted to our hospital or outpatient from February 2021 to December 2021 were included in the study.According to the random number table,patients were divided into different groups of control or Dapagliflozin groups,control group treated with optimized drugs,dapagliflozin group treated with optimized drugs and Dapagliflozin.All patients were treated for 60 days at least,ProBNP,dilated left ventricular diameters(LDV),left ventricular ejection fraction(LVEF),Aldosterone and 6MWT were measured two times at least.In the end,total 101 patients were included.Results The therapeutic effects in dapagliflozin group were all significant compared with control group.Conclusion Dapagliflozin can effectively improve the treatment effect of patients with congestive heart failure with reduced ejection fraction and it probably connected with the inhibition of RAAS.
