摘  要:目的探讨孕期给予个体化营养膳食指导对孕妇膳食结构及母婴结局的影响。方法根据孕妇是否选择孕期个体化营养膳食指导将2019年1月至2020年12月在南通市妇幼保健院产科门诊建立孕产妇保健手册的400名孕妇分为对照组和观察组。排除失访者,对照组有176名孕妇纳入研究,定期接受孕期常规保健;观察组有210名孕妇纳入研究,接受个体化营养膳食指导。比较两组孕妇孕期增重情况和符合增重推荐标准的达标比例、孕期各类食物摄入量符合推荐标准的达标比例及母婴结局。结果观察组孕期体质量增长低于对照组,符合增重推荐标准的达标比例高于对照组,差异有统计学意义(P<0.01);观察组孕期谷类、蔬菜、水果、蛋类、豆类及油脂类摄入量符合推荐标准的达标比例高于对照组(P<0.05),肉类和奶类摄入量符合推荐标准的达标比例比较,差异无统计学意义(P>0.05);观察组妊娠期贫血、剖宫产、巨大儿发生率低于对照组(P<0.05),妊娠期高血压疾病、妊娠期糖尿病、产后出血、新生儿窒息、早产发生率比较,差异无统计学意义(P>0.05)。结论孕期实施个体化营养膳食指导对改善孕妇膳食结构及母婴结局具有重要价值。Objective To explore the effect of individualized nutrition dietary guidance during pregnancy on dietary pattern,maternal and infant outcomes of pregnant women.Methods A total of 400 pregnant women who established maternal health handbook in Obstetrics Outpatient Clinic of Nantong Maternal and Child Health Care Hospital from January 2019 to December 2020 were selected for this study.They were divided into the control group and the observation group according to whether pregnant women volunteered to choose individualized nutritional dietary guidance during pregnancy.Excluding lost visitors,there were 176 pregnant women in the control group,and 210 pregnant women in the observation group.The control group received routine prenatal care,while the observation group received individualized nutrition dietary guidance.The weight gain during pregnancy,the proportion meeting the recommended weight gain standards,the proportion meeting the recommended food intake standards during pregnancy,maternal and infant outcomes were compared between the two groups.Results The weight gain during pregnancy in the observation group was lower than that in the control group,and the proportion of meeting the recommended weight gain standard was higher than that in the control group,with significant differences(P<0.01).The proportions of intake of grains,vegetables,fruits,eggs,legumes and oils meeting the recommended standards in the observation group were higher than those in the control group(P<0.05),and there were no statistical differences in the proportions of intake of meat and milk which met the recommended standards(P>0.05).The incidence rates of pregnancy anemia,cesarean section and macrosomia in the observation group were lower than those in the control group(P<0.05),and there were no differences in the incidence rates of pregnancy hypertension,gestational diabetes,postpartum hemorrhage,neonatal asphyxia and preterm birth(P>0.05).Conclusion Individualized nutrition dietary guidance during pregnancy can effectively improve the die
