袁青分期辨治痛风性关节炎经验

摘　　要：痛风性关节炎是临床常见的尿酸代谢异常引起的痛风性关节炎,针灸治疗本病具有疗效好、不良反应少等优势。袁青教授认为本病脾肾两虚为本,痰湿浊瘀痹阻骨节为标,病情复杂,主张分期辨治:急性期属湿热蕴结证,针刺手三针(合谷、曲池、外关)、足三针(足三里、三阴交、太冲)以清热除湿、通络止痛;间歇期责于痰湿化浊,清浊相干,营卫失调,治以“导气同精法”升清降浊,调和营卫;慢性期基本病机为脾肾两虚,浊邪从寒化瘀,以灸法健脾益肾,温阳化瘀。Gouty arthritis is a common clinical condition caused by abnormal uric acid metabolism,and acupuncture treatment has the advantages of good efficacy and less adverse reactions.Professor Yuan Qing believes that the disease is caused by deficiency of spleen and kidney,and the symptoms are complicated by phlegm,dampness,turbidity and stagnation of the bones and joints.In the acute stage,it is dampness and heat,so acupuncture the three needles of the hand(Hegu,Quchi and Waiguan) and the three needles of the foot(Sansanli,Sanyinjiao and Taichong) to clear heat and remove dampness,pass the ligaments and relieve pain;in the interval,it is due to the phlegm and dampness transforming into turbidity,the clear and turbidity are intertwined,and the nutrient qi and defensive qi are out of balance,so the treatment is to "Dao qi Tongjing method" to raise the clear and lower the turbidity,and harmonize the nutrient qi and defensive qi.In the chronic stage,the basic pathogenesis is the deficiency of both spleen and kidney,and the turbid evil transforms into stasis from cold,so moxibustion is used to strengthen the spleen and kidney,and warm the yang to remove stasis.
