黑色胸腔积液1例报告并文献复习

摘　　要：目的探讨黑色胸腔积液的诊断和鉴别诊断。方法对1例单侧大量胸腔积液由暗红色转为黑色的罕见临床病例资料进行分析。以“black pleural effusion”“black pleural fluid”为关键词在Pubmed、Wed of Science数据库检索黑色胸腔积液相关文献,以“黑色胸腔积液”“黑色胸水”为关键词检索万方医学网和中国知网数据库相关文献,检索截止时间为2022年3月。结果病人胸腔积液由暗红色转为黑色,结合酗酒史、胸腔积液渗出液性质、胸腔积液淀粉酶升高、胸腹部CT及重建图像,最终诊断为胰性胸腔积液、继发于胰腺假性囊肿。检索到相关中英文文献25篇(均为病例报告),共26例病人。结论拓宽对黑色胸腔积液病因谱的认识,结合规范的临床思维,有利于该类疾病早期诊断。Objective To investigate the diagnosis and differential diagnosis of black pleural effusion.Methods An analysis was performed for the clinical data of a rare case of unilateral massive pleural effusion changing from dark red to black.Databases including PubMed,Wed of Science,Wanfang Data,and CNKI were searched for the articles on black pleural effusion published up to March 2022 with the keywords of“black pleural effusion”and“black pleural fluid”in English and Chinese.Results The pleural effusion of the patient changed from dark red to black,and the patient was finally diagnosed with pancreatic pleural effusion secondary to pancreatic pseudocyst based on the history of drinking,the nature of pleural effusion,the elevation of amylase in pleural effusion,and chest and abdominal CT with reconstruction images.A total of 25 case reports were obtained,with 26 patients in total.Conclusion Broadening the understanding of the etiology spectrum of black pleural effusion combined with stan-dardized clinical thinking may help with the early diagnosis of such disease.
