石氏加味逐痰通络汤治疗腰椎间盘突出症(痰瘀阻络证)的临床观察

摘　　要：目的 评价石氏加味逐痰通络汤治疗腰椎间盘突出症(痰瘀阻络证)的临床疗效及安全性。方法 纳入腰椎间盘突出症(痰瘀阻络证)患者100例,随机数字表法分为两组。对照组50例口服塞来昔布胶囊,观察组50例口服加味逐痰通络汤,观察治疗1、14、28 d时JOA腰腿痛评分、VAS视觉评分,评估JOA腰腿痛治疗改善率、VAS疼痛率改善率;记录不良反应。结果 治疗1 d,两组JOA、VAS分值比较无显著差异(P>0.05);治疗14、28 d两组JOA分值均提高且观察组高于对照组,两组VAS分值均降低且观察组低于对照组(P<0.05)。观察组治疗14 d时JOA改善率、VAS改善率均高于对照组,观察组治疗28 d时JOA改善率、VAS改善率均高于对照组(P<0.05)。两组均无明显不良反应。结论 石氏加味逐痰通络汤治疗腰椎间盘突出症(痰瘀阻络证)效果较常规西药更好,能进一步缓解疼痛,提高腰椎功能,且不良反应少,安全性高。Objective:To evaluate the clinical efficacy and safety of Shishi Jiawei Zhutan Tongluo Decoction in the treatment of phlegm and blood stasis blocking lumbar disc herniation. Methods:A total of 100 patients with phlegm and blood stasis obstructing collateral type lumbar intervertebral disc herniation in the authors′ hospital were enrolled. They were divided into two groups by random number table. The control group(50 cases)was treated with oral celecoxib capsules,and the observation group(50 cases)was treated with oral administration of Shishi Jiawei Zhutan Tongluo Decoction. The improvement rates of JOA low back pain score and VAS score at 1,14,28d after treatment were observed;the adverse reactions were recorded. Results:After 1 day of treatment,the JOA score and VAS score of the two groups were compared and showed no significant differences(P >0.05);at the 14and 28 d,the JOA score of the two groups increased and the observation group was higher than the control group,and the VAS scores of the two groups decreased and the observation group was lower control group(P< 0.05). The JOA improvement rate and VAS improvement rate of the observation group were higher than those of the control group at 14 d of treatment;the JOA improvement rate and VAS improvement rate of the observation group were higher than those of the control group at 28 d of treatment(P< 0.05). There were no obvious adverse reactions in both groups. Conclusion:Shishi Jiawei Zhutan Tongluo Decoction is more effective in treating phlegm-stasisblocking lumbar disc herniation than conventional western medicine. It can further relieve pain and improve lumbar function,with few adverse reactions and high safety,which is worthy of promotion.
