宽胸理肺汤合三子养亲汤对COPD急性加重期患者气道重塑及血清降钙素原、C反应蛋白的影响

摘　　要：目的 探讨宽胸理肺汤合三子养亲汤对COPD急性加重期(AECOPD)患者气道重塑及血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)的影响。方法 选取120例患者随机分为对照组与观察组各60例。对照组行常规吸氧、祛痰、平喘、抗感染对症支持治疗,观察组在对照组基础上联合宽胸理肺汤合三子养亲汤治疗。比较治疗前后两组患者中医证候积分、肺功能指标、炎症因子指标以及气道重塑指标。比较两组患者中医证候疗效。结果 观察组、对照组总有效率分别为96.67%、86.67%,观察组显著高于对照组(P<0.05)。治疗后观察组主症双肺啰音、喘息气短与中医证候积分低于对照组(P<0.05);1秒用力呼气量(FEV1)、呼吸峰流速(PEF)、气道峰压(Ppeak)水平高于对照组(P<0.05);白细胞介素-17(IL-17)、PCT、hs-CRP、碱性成纤维细胞生长因子(b-FGF)、基质金属蛋白酶-9(MMP-9)、内皮生长因子(VEGF)水平低于对照组(P<0.05)。结论 宽胸理肺汤合三子养亲汤对COPD急性加重期患者疗效确切,可降低血清PCT、CRP等炎症指标水平,改善患者肺功能与临床症状,有效抑制气道重塑。Objective:To investigate the effects of Kuanxiong Lifei Decoction and Sanzi Yangqin Decoction on airway remodeling and serum procalcitonin(PCT)and C-reactive protein(CRP)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods:A total of 120 patients with AECOPD were randomly divided into the control group and the observation group,60 cases in each group. The control group was given routine oxygen inhalation,expectorant,anti-asthmatic,anti-infection symptomatic and supportive treatment,and the observation group was given the combination of Kuanxiong Lifei Decoction and Sanzi Yangqin Decoction on the basis of the control group,and the curative effect was observed after one week of treatment. The TCM syndrome scores,pulmonary function indexes,inflammatory factor indexes and airway remodeling indexes were compared between the two groups before and after treatment. The therapeutic effects of TCM syndromes were compared between the two groups. Results:The total effective rates of the observation group and the control group were 96.67% and 86.67%,respectively,and the observation group was significantly higher than the control group(P< 0.05). The main symptoms of bilateral lung rales,shortness of breath and TCM syndrome scores in the observation group were lower than those in the control group(P< 0.05). The levels of FEV1,PEF and Ppeak in the observation group were higher than those in the control group(P< 0.05). The levels of IL-17,PCT,hs-CRP,b-GFG,MMP-9 and VEGF in the observation group were lower than those in the control group(P< 0.05). Conclusion:Kuanxiong Lifei Decoction combined with Sanzi Yangqin Decoction has an exact curative effect on patients with AECOPD. It can reduce the levels of serum PCT,CRP and other inflammatory indicators,improve the lung function and clinical symptoms of patients,and effectively inhibit airway remodeling.
