电针对前循环脑梗死患者神经功能康复及血清NLRP3、IL-18水平的影响

摘　　要：目的观察电针对急性期前循环脑梗死(ACI)患者神经功能康复及血清NOD样受体热蛋白结构域3(NLRP3)、白介素-18(IL-18)水平的影响。方法将59例急性期ACI患者,随机分为治疗组(30例)和对照组(29例)。对照组予以常规治疗,治疗组在常规治疗基础上给予电针治疗。观察两组治疗前后血清NLRP3、IL-18水平和美国国立卫生研究院卒中量表(NIHSS)、简化Fugl-Meyer运动功能评定法(SFMA)、改良Barthel指数(MBI)评分的变化。结果治疗后,两组NIHSS、SFMA和MBI评分优于治疗前,差异有统计学意义(P<0.01);治疗组NIHSS和SFMA评分优于对照组(P<0.05);两组MBI评分比较,差异无统计学意义(P>0.05)。治疗后,两组血清NLRP3和IL-18水平低于治疗前(P<0.01);治疗组NLRP3和IL-18水平明显低于对照组(P<0.05)。结论在常规治疗基础上,电针可能通过下调血清中NLRP3和IL-18水平,进一步促进急性期ACI患者神经功能的修复。Objective To observe the effects of electroacupuncture on neurological rehabilitation and serum levels of NOD-like receptor thermal protein domain associated protein 3(NLRP3)and interleukin-18(IL-18)in patients with anterior cerebral infarction(ACI)in the acute stage.Method Fifty-nine patients in the acute stage of ACI were randomized into a treatment group(30 cases)and a control group(29 cases).The control group was given conventional intervention,and the treatment group received additional electroacupuncture treatment.The serum NLRP3 and IL-18 levels and the scores of the National Institute of Health stroke scale(NIHSS),simplified Fugl-Meyer assessment(SFMA),and modified Barthel index(MBI)were observed before and after the treatment.Result After the treatment,the NIHSS,SFMA,and MBI scores improved in both groups,showing statistical significance(P<0.01);the treatment group surpassed the control group in comparing the NIHSS and SFMA scores(P<0.05),while there was no significant between-group difference in comparing the MBI score(P>0.05).The serum NLRP3 and IL-18 levels dropped after the intervention in the two groups(P<0.01)and were markedly lower in the treatment group than in the control group(P<0.05).Conclusion Based on conventional treatment,electroacupuncture may promote the repair of neurological function of patients in the acute stage of ACI by down-regulating the serum levels of NLRP3 and IL-18.
