摘　　要：目的分析精子DNA碎片化指数(DFI)对其配偶体外受精-胚胎移植(IVF-ET)后胚胎质量的影响。方法选取2020年3月至2021年3月漯河市中心医院接收的139例行IVF-ET术者为研究对象,依据DFI分为低含量组(DFI<10%,n=37)、中含量组(10%≤DFI<20%,n=76)、高含量组(DFI≥20%,n=26),对比3组精子常规参数(精子浓度、精子活力、前向运动精子数)、胚胎质量(优质胚胎率、种植成功率、受精成功率)、妊娠结局(流产率、临床妊娠率、活产率、早产率)。结果低含量组、中含量组前向运动精子数高于高含量组(P<0.05);低含量组受精成功率、种植成功率、优质胚胎率高于中含量组和高含量组,且中含量组受精成功率、种植成功率、优质胚胎率高于高含量组(P<0.05);低含量组临床妊娠率、活产率高于中含量组与高含量组,且中含量组临床妊娠率、活产率高于高含量组(P<0.05)。结论DFI越低,配偶IVF-ET后受精成功率、种植成功率、临床妊娠率、活产率越高,临床可通过干预DFI来改善妊娠结局。Objective To analyze the effect of the sperm DNA fragmentation index(DFI)on the embryo quality after in vitro fertilization-embryo transfer(IVF-ET).Methods A total of 139 patients of IVF-ET received by Luohe Central Hospital from March 2021 to March 2022 were selected as the research objects.According to DFI,they were divided into low content group(DFI<10%,n=37),medium content group(10%≤DFI<20%,n=76)and high content group(DFI≥20%,n=26).The conventional parameters of sperm(sperm concentration,sperm vitality,forward moving sperm number),embryo quality(quality embryo rate,implantation success rate,fertilization success rate)and pregnancy outcome(abortion rate,clinical pregnancy rate,live birth rate,premature birth rate)were compared among the three groups.Results The forward moving sperm number in low and medium content groups was higher than that in high content groups(P<0.05).The fertilization success rate,implantation success rate and high-quality embryo rate of low content group were higher than those of medium content group and high content group,and the fertilization success rate,implantation success rate and high-quality embryo rate of medium content group were higher than those of high content group(P<0.05).The clinical pregnancy rate and live birth rate of low content group were higher than those of medium content group and high content group,and the clinical pregnancy rate and live birth rate of medium content group were higher than those of high content group(P<0.05).Conclusion The lower the DFI,the higher the successful rate of fertilization,implantation,clinical pregnancy rate and live birth rate after IVF-ET.The pregnancy outcome can be improved by intervention of DFI.
