2D-STI在左室射血分数保留的完全性左束支传导阻滞患者心室功能中的评估价值

摘　　要：目的探讨二维斑点追踪成像(2D-STI)在左室射血分数(LVEF)保留的完全性左束支传导阻滞(CLBBB)患者心室功能评估中的价值。方法选择2019年6月至2021年12月河南科技大学第一附属医院心内科收治的84例LVEF保留的CLBBB患者作为研究对象,依据QRS波时间分为宽QRS组(QRS>150 ms,n=45)、窄QRS组(120 ms≤QRS≤150 ms,n=39),另选取同期在医院行健康体检的40例志愿者作为健康对照组。3组患者均行2D-STI技术检查,比较3组心室功能[左心房舒张末期内径(LVEDD)、左心房前后径(LAD)、LVEF、二尖瓣口舒张早期及舒张晚期血流速度(E峰和A峰)、二尖瓣环运动速度(e’),E/A及E/e’比值、左心室整体纵向应变(LVGLS)、右心室整体纵向峰值应变(RVGLS)、室间隔整体纵向应变(LS-Sept)、左心室17节段纵向峰值应变达峰时间标准差(SDt)],并分析2D-STI技术对LVEF保留的CLBBB患者心室功能的评估价值。结果宽QRS组LVEDD、LVEF、LAD值最高,其次为窄QRS组、健康对照组;窄QRS组A峰、E/e’值最高,其次为宽QRS组、健康对照组,健康对照组E峰、E/A、e’值最高,3组间比较,差异有统计学意义(P<0.05)。宽QRS组LVGLS、RVGLS、LS-Sept最低,其次为窄QRS组、健康对照组,SDt、QRS波时限最高,其次为窄QRS组、健康对照组,3组间比较,差异有统计学意义(P<0.05)。一般双变量Peason相关性分析证实,LVGLS、RVGLS、LS-Sept与QRS波时限呈负相关(r<0,P<0.05),SDt与QRS波时限呈正相关(r>0,P<0.05)。结论LVEF保留的CLBBB患者心室收缩、舒张功能及收缩同步性减低,2D-STI技术对LVEF保留的CLBBB患者心室功能具有一定的评估价值。Objective To investigate the value of 2-dimensional speckle tracking imaging(2D-STI)in assessing ventricular function in patients with complete left bundle branch block(CLBBB)with preserved left ventricular ejection fraction(LVEF).Methods A total of 84 patients with CLBBB with preserved LVEF treated in the Department of Cardiology of the First Affiliated Hospital of Henan University of Science and Technology from June 2019 to December 2021 were selected as the research subjects.According to QRS wave time,they were divided into wide QRS group(QRS>150 ms,n=45)and narrow QRS group(120 ms≤QRS≤150 ms,n=39).Another 40 volunteers who underwent physical examination in the hospital in the same period were selected as the healthy control group.All patients in the three groups underwent 2D-STI.Ventricular function[left atrial end diastolic diameter(LVEDD),left atrial anterior posterior diameter(LAD),LVEF,early and late diastolic blood flow velocity of mitral valve orifice(peak E and peak A),mitral annular motion velocity(e’),E/A and E/e’ratios,left ventricular global longitudinal strain(LVGLS),right ventricular global longitudinal peak strain(RVGLS),ventricular septal global longitudinal strain(LS-Sept)and standared deviation of left ventricle 17 segments peak strain time(SDt)]were compared among the three groups,and the value of 2D-STI technology in the evaluation of ventricular function in CLBBB patients with preserved LVEF was analyzed.Results LVEDD,LVEF and LAD were the highest in wide QRS group,followed by narrow QRS group and healthy control group,the values of peak A and E/e’in narrow QRS group were the highest,followed by wide QRS group and healthy control group,the values of peak E,E/A and e’in healthy control group were the highest,and there was statistical significant difference among the three groups(P<0.05).LVGLS,RVGLS and LS-Sept were the lowest in wide QRS group,followed by narrow QRS group and healthy control group,with the highest SDt and QRS wave duration,followed by narrow QRS group and health
