双相气道正压通气呼吸机联合纳洛酮对老年慢性阻塞性肺疾病合并呼吸衰竭患者的疗效

摘　　要：目的分析双相气道正压通气呼吸机(BiPAP)联合纳洛酮治疗老年慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的效果。方法选取2016年1月至2019年12月在南阳市第一人民医院治疗的100例老年COPD合并呼吸衰竭患者为研究对象,采用随机数表法将患者分为对照组和观察组,各50例。对照组接受盐酸纳洛酮注射液静脉泵注治疗,观察组在此基础上应用BiPAP辅助治疗。比较两组治疗前后肺功能和动脉血气指标变化。结果治疗前,两组肺功能和动脉血气指标差异无统计学意义(P>0.05)。治疗后,两组肺活量、深吸气量、残气量较治疗前均升高,残气量较治疗前降低(P<0.05)。观察组肺活量、深吸气量、残气量、最大呼气中期流速高于对照组,残气量低于对照组(P<0.05)。两组动脉血氧分压、pH值较治疗前升高,观察组高于对照组(P<0.05)。两组二氧化碳分压较治疗前降低,观察组低于对照组(P<0.05)。在治疗过程中,对照组不良反应发生率(14.00%)和观察组(10.00%)比较,差异无统计学意义(P>0.05)。结论对于老年COPD合并呼吸衰竭患者,BiPAP联合纳洛酮治疗有助于促进肺功能恢复,改善动脉血气指标。Objective To analyze the effect of bi-level positive airway pressure ventilator(BiPAP)combined with naloxone in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Methods One hundred elderly patients with COPD and respiratory failure treated in Nanyang First People’s Hospital from January 2016 to December 2019 were selected as research objects.The patients were randomly divided into control group and observation group,with 50 cases in each group.Control group was treated with naloxone hydrochloride injection by intravenous pump,and observation group was treated with BiPAP on this basis.The changes of pulmonary function and arterial blood gas were compared between the two groups before and after treatment.Results Before treatment,there was no statistical difference in pulmonary function and arterial blood gas between the two groups(P>0.05).After treatment,vital capacity,inspiratory capacity and residual capacity of both groups were higher than before treatment,and residual capacity was lower than before treatment(P<0.05).The vital capacity,deep inspiratory capacity,residual volume and maximum mid expiratory flow rate in observation group were higher than those in control group,and the residual volume was lower than that in control group(P<0.05).The arterial partial pressure of oxygen and pH in the two groups were higher than those before treatment,and those in observation group were higher than those in control group(P<0.05).The partial pressure of carbon dioxide in the two groups was lower than that before treatment,and that in observation group was lower than that in control group(P<0.05).In the course of treatment,there was no statistical difference in the incidence of adverse reactions between control group(14.00%)and observation group(10.00%)(P>0.05).Conclusion For elderly COPD patients with respiratory failure,BiPAP combined with naloxone is helpful to promote the recovery of pulmonary function and improve arterial blood gas indicators.
