沙库巴曲缬沙坦与赖诺普利对射血分数保留心力衰竭患者的疗效对比  

Efficacy of Sacubitril Valsartan Sodium Tablets and Lisinopril in Patients with Ejection Fraction Preserved Heart Failure

摘　　要：目的探讨沙库巴曲缬沙坦与赖诺普利对射血分数保留心力衰竭(HFpEF)患者的临床疗效差异。方法将2020年5月至2021年6月商丘市中心医院收治的75例HFpEF患者按照随机数表法分为观察组(38例)与对照组(37例)。对照组在常规治疗的基础上加用赖诺普利,观察组在常规治疗的基础上加用沙库巴曲缬沙坦,连续治疗1个月。对比两组患者用药后的临床疗效。结果对照组最终有效率为62.16%,观察组为84.21%,与对照组对比,观察组的最终有效率更高(P<0.05)。治疗后,观察组血清氨基末端脑利钠肽前体(NT-proBNP)水平低于对照组,左室射血分数(LVEF)高于对照组(P<0.05)。两组患者6分钟步行距离试验(6MWD)均大于治疗前,且观察组长于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论相比赖诺普利,应用沙库巴曲缬沙坦能更好地控制HFpEF患者临床症状,降低血清NT-proBNP水平,提高患者心功能,且药物安全可靠。Objective To compare the clinical efficacy between sacubitril valsartan sodium tablets and lisinopril in the treatment of ejection fraction preserved heart failure(HFpEF).Methods Seventy-five patients with HFpEF treated in Shangqiu Central Hospital from May 2020 to June 2021 were selected,and divided into two groups according to the random number table methods.Both groups received routine treatment,based on this,control group(37 cases)received lisinopril,while observation group(38 cases)received sacubitril valsartan sodium tablets.Treatment in both groups lasted for 1 month.Then the clinical response rate was compared between the two groups.Results The final effective rate was 62.16%in the control group and 84.21%in the observation group.Compared with the control group,the final effective rate was higher in the observation group(P<0.05).After treatment,serum N-terminal pro-B-type natriuretic peptide level(NT-proBNP)of the observation group was lower than that of the control group,and the left ventricular ejection fraction(LVEF)was higher than that of the control group(P<0.05).After treatment,the 6 minute walking distance(6 MWD)test between both groups were prolonged and the observed group was larger than the control group(P<0.05).There was no statistical difference on incidence of adverse effects between the two groups(P>0.05).Conclusion Compared with lisinopril treatment,application of sacubitril valsartan sodium tablets can effectively ameliorate the clinical symptoms,reduce the serum NT-proBNP level and improve cardiac function in a more reliable and safe way.
