摘　　要：目的:评估盆底超声技术对女性排便协同失调(dyssynergic defecation,DD)的临床诊断价值。方法:选取2019年1月—2020年12月南京市中医院肛肠科收治的女性排便协同失调患者40例,设为DD组;另选取同期无症状人群35名为无症状组。采用经会阴和经肛门三维超声分别测量两组的肛直角、耻骨直肠肌角。结果:无症状人群Valsalva动作时的肛直角和耻骨直肠肌角均显著大于静息状态(P<0.05);排便协同失调患者Valsalva动作时肛直角和耻骨直肠肌角均显著小于静息状态(P<0.01)。Valsalva动作时两组肛直角变化、耻骨直肠肌角均有显著差异(P<0.01)。结论:肛直角和耻骨直肠肌角分别可以作为经会阴和经肛门三维超声诊断排便协同失调的重要影像学指标。Objective To evaluate the diagnostic value of pelvic floor ultrasound in women with dyssynergic defecation(DD).Methods A total of 40 female patients with dyssynergic defecation admitted to the Department of Anorectal,Nanjing Hospital of Traditional Chinese Medicine from January 2019 to Decemer 2020 were selected as the DD group;another 35 asymptomatic people during the same period were selected as the asymptomatic group.Both groups received transperineal ultrasound(TPUS) and threedimensional endoanal ultrasound(3D-EAUS).Anorectal angle,puborectalis angle were measured.Results In asymptomatic people,the anorectal angle and the puborectalis angle during Valsalva were both significantly greater than that in resting state(P <0.05);In patients with DD,the anorectal angle and the puborectalis angle during Valsalva were significantly smaller than that in the resting state(P<0.01).There were significant differences in the changes of the anorectal angles and the puborectalis angles between the two groups during the Valsalva maneuver(P <0.01).Conclusion The anorectal angle and puborectalis angle can be used as important imaging indicators for the diagnosis of DD by TPUS and 3D-EAUS.
