摘  要:目的:总结大脑镰旁和小脑幕下硬膜下血肿的CT影像特征,提高大脑镰旁和小脑幕下硬膜下血肿的诊断准确率。方法:回顾性分析上海市松江区泗泾医院2019年3月—2021年3月间收治的27例经手术证实为大脑镰旁及小脑幕下硬膜下血肿患者的颅脑CT平扫图像,查阅文献资料。结果:大脑镰旁硬膜下血肿CT平扫征象为沿大脑镰局限或弥漫分布的条状高密度影,血肿大脑镰邻接面边缘平直,脑组织邻接面呈弧形或波浪状;小脑幕下硬膜下血肿CT平扫征象为岩锥以上层面小脑幕处局限性条片状高密度影,其外侧缘边界清晰,内侧缘边界略模糊,MPR冠状位重建表现为与小脑幕紧密相连的镰状高密度影像。结论:大脑镰旁及小脑幕下硬膜下血肿CT平扫征象具有诊断意义,仔细分析CT平扫图像征象可以准确诊断。Objective To summarize the CT image features of parafalx and infratentorial subdural hematoma,and to improve the diagnostic accuracy of parafalx and infratentorial subdural hematoma.Methods The CT plain scan images of 27 patients with subdural hematomas near the falx cerebri and under the tentorium cerebelli confirmed by surgery from March 2019 to March 2021 in Sijing Hospital were retrospectively analyzed,and the literature was consulted.Results The CT findings of subdural hematoma near cerebral falx were strip-shaped high-density shadow distributed locally or diffusely along cerebral falx.The edge of the adjacent surface of cerebral falx was straight,and the adjacent surface of brain tissue was arc or wave;The CT findings of subdural hematoma under the cerebellar tentorium were localized strip-shaped high-density shadow at the cerebellar tentorium above the petrous cone.The boundary of the outer lateral edge was clear and the boundary of the medial edge was slightly blurred.The coronal reconstruction of MPR showed sickle shaped high-density image closely connected with the cerebellar tentorium.Conclusion The CT findings of parafalx and infratentorial subdural hematoma have diagnostic significance.Careful analysis of CT findings can accurately diagnose it.
