摘  要:目的评估血清细胞因子在肺炎支原体肺炎儿童中起的作用。方法257例因社区获得性肺炎住院的儿童纳入本研究并分为2组:MPP组和非MPP组。使用ELISA法测MP-IgM和MP-IgG来确立MP感染。使用流式细胞法测定血清白介素-2(IL-2)、IL-4、IL-6、IL-10、肿瘤坏死因子α(TNF-α)和干扰素γ(IFN-γ)。通过倾向评分匹配分析配对75对儿童校正混杂因素如年龄、发热天数、喘息、影像学特征。结果MPP组儿童和非MPP组之间的血清IL-2、IL-4、TNF-α和IFN-γ水平无统计学差异。与非MPP组相比,MPP组儿童的血清IL-6水平显著升高,血清IL-10水平显著降低。通过倾向评分匹配后,血清IL-10水平的这种统计学差异仍然存在。结论MPP儿童血清IL-10水平显著降低,可能提示IL-10在MPP发病机制中起的作用。Objective To evaluate the role of the serum cytokines in children with mycoplasma pneumoniae pneumonia(MPP).Methods 257 Children hospitalized due to community-acquired pneumonia(CAP)were enrolled and divided into 2 groups:MPP group and non-MPP group.MP IgM and MP IgG were measured by ELISA method to confirm MP infection.Serum levels of interleukin-2(IL-2),IL-4,IL-6,IL-10,tumor necrosis factor-α(TNF-α),and interferon-γ(IFN-γ)were measured by flow cytometry.Furthermore,we used 75 paired patients in a propensity score matching analysis to correct for confounding factors such as age,days of fever,wheezing,radiology features.Results There was no significant difference in serum level of IL-2,IL-4,TNF-αand IFN-γbetween children with MPP and those with non-MPP.A significantly higher level of IL-6 and lower level of IL-10 was found in children with MPP than in children with non-MPP.In the propensity score matching,we observed that significant differences of the serum level of IL-10 was still present.Conclusion The serum level of IL-10 in children with MPP decreased significantly,which may suggest a effect of IL-10 in the pathogenesis of MPP.
