基于“双心理论”的女性冠心病合并精神心理障碍发病机制及临床研究概况

摘　　要：现代医学对女性双心疾病的发病机制研究主要有神经内分泌学说、炎症反应学说、内皮功能紊乱、血小板功能障碍、雌激素撤退假说等,临床主要给予患者相应的心理干预或抗精神药物治疗。中医认为本病的发病机制为气血失调,脏腑失衡,基于“双心理论”并结合女性的生理病理特点,重视从“心”而论,五脏相关,多脏同治,辨证运用养心安神、疏肝解郁、滋养肝肾、交通心肾等治则,采取外治疗法、运动疗法、情志相胜疗法、认知行为疗法等。应提高临床医生对女性心血管疾病患者精神心理问题的关注,多脏同治,多法并举,充分发挥中西医结合治疗双心疾病的优势。参考文献35篇。Modern medicine believes that the pathogenesis of female psycho-cardiological disease mainly includes neuroendocrine theory,inflammatory response theory,endothelial dysfunction,platelet dysfunction,and estrogen withdrawal hypothesis,etc.,and patients are mainly given corresponding psychological intervention or antipsychotic treatment.Traditional Chinese medicine believes that the pathogenesis of this disease is imbalance of qi and blood and disharmony of zangfu-viscera.Based on the“psycho-cardiological theory”and combined with the physiological and pathological characteristics of women,attention should be paid to the“heart”.The five zang-viscera are interrelated,and multiple zang-viscera are treated together.The treatment principles of nourishing heart and tran quilizing mind,soothing liver and relieving depression,nourishing liver and kidney,coordinating heart and kidney,and external treatment,exercise therapy,emotional therapy,cognitive behavioral therapy,etc.are applied according to syndrome differentiation.Clinicians should pay more attention to the mental and psychological problems of female patients with cardiovascular disease,treat multiple zang-viscera at the same time,and use multiple methods simultaneously,so as to give full play to the advantages of integrated traditional Chinese and Western medicine in the treatment of psycho-cardiological disease.There are 35 references in total.
