中医药治疗良性前列腺增生术后尿失禁临床研究概况

摘　　要：良性前列腺增生术后尿失禁病位在膀胱,与肾、脾、三焦等脏腑关系密切;病机主要为气虚,肾与膀胱气化不利,固摄尿液失司。老年男性脏腑气血虚弱,加之前列腺切除术易伤及元气,临床多为肾虚证、脾虚证,治疗上多以“虚则补之”为大法,治法有益肾活血通淋、补肾益气固涩、补中益气健脾等。近年来良性前列腺增生术后尿失禁的中医药治疗方法,主要包括中药口服、针灸、针药联合、中西医结合及耳穴贴敷等,疗效显著,具有独特优势;也存在某些局限性,如中医辨证分型及疗效评价标准不统一,科研设计方案和统计学方法不够严谨,缺乏大样本、多中心随机对照试验等。参考文献34篇。Postoperative urinary incontinence of benign prostatic hyperplasia lies in bladder,closely related to kidney,spleen and triple energizer.The pathogenesis mainly includes:qi deficiency,disturbance of qi transformation of kidney and bladder failing to secure urination.Elderly men are deficient in qi and blood of zangfu-viscera and the prostatectomy is easy to damage the original qi,causing kidney deficiency and spleen deficiency in the clinic.In terms of treatment,the main treatment principle is“treating deficiency with tonification”,methods including:replenishing kidney,activating blood and relieving stranguria,tonifying kidney,replenishing qi and arresting discharge,invigorating spleen-stomach and replenishing qi and invigorating spleen,etc.In recent years,traditional Chinese medicine treatment methods for postoperative urinary incontinence of benign prostatic hyperplasia mainly include oral administration of traditional Chinese medicine,acupuncture and moxibustion,acupuncture and medicine combination,integrated traditional Chinese and western medicine,and auricular point sticking,with distinct curative effect and unique advantage.There still exist some limitations such as:syndrome classification and efficacy evaluation standards are not uniform,not rigorous scientific research design and statistical method,and lack of large sample and multi-center randomized controlled trials.There are 34 references in total.
