急性心肌梗死患者血清GMP-140、CD63、CD62P的表达水平及其与心功能的相关性

摘　　要：目的观察急性心肌梗死患者血清颗粒膜蛋白140(GMP-140)、溶酶体膜糖蛋白(CD63)、P选择素(CD62P)的表达水平,并探讨其与心功能的相关性。方法选择西安市中医医院2019年1月至2021年1月收治的100例急性心肌梗死患者作为观察组,并选择同期在我院接受体检的100例健康人员作为对照组。比较两组受检者纳入研究后次日血清GMP-140、CD63、CD62P水平及左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD),并比较观察组不同心功能分级患者的血清GMP-140、CD63、CD62P表达水平,采用Pearson相关性分析法分析血清GMP-140、CD63、CD62P水平与心功能指标的相关性。结果观察组患者的血清GMP-140、CD63、CD62p水平、LVEDD、LVESD分别为(53.18±6.22)ng/mL、(13.72±2.69)%、(14.89±2.77)%、(55.72±4.81)mm、(53.92±5.06)mm,明显高(厚)于对照组的(27.83±3.17)ng/mL、(4.33±0.72)%、(3.56±0.61)%、(45.69±3.42)mm、(34.12±3.28)mm,而LVEF为(45.82±4.75)%,明显低于对照组的(69.12±4.29)%,差异均有统计学意义(P<0.05);观察组患者心功能分级Ⅳ级患者的血清GMP-140、CD63、CD62P水平分别为(63.79±5.69)ng/mL、(18.35±2.62)%、(20.12±2.71)%,明显高于Ⅲ级患者的(56.31±5.12)ng/mL、(14.28±2.05)%、(16.33±2.18)%,Ⅱ级患者的(49.82±4.21)ng/mL、(11.52±2.17)%、(12.16±2.40)%,以及Ⅰ级患者的(39.64±3.76)ng/mL、(9.04±1.42)%、(8.76±1.68)%,且Ⅲ级患者明显高于Ⅱ级患者,Ⅱ级患者明显高于Ⅰ级患者,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,血清GMP-140、CD63、CD62P与LVEF均呈负相关(P<0.05),与LVEDD、LVESD均呈正相关(P<0.05)。结论急性心肌梗死患者血清GMP-140、CD63、CD62P水平均明显升高,且与心功能指标具有相关性,临床上应予以重视。Objective To observe the expression of the serum granulosa membrane protein 140(GMP-140),lysosomal membrane glycoprotein(CD63),P-selectin(CD62P)levels in patients with acute myocardial infarction,and to discuss their correlation with cardiac function.Methods A total of 100 patients with acute myocardial infarction admitted to Xi'an Hospital of Traditional Chinese Medicine from January 2019 to January 2021 were selected as the observation group,and 100 healthy persons receiving physical examination in the hospital during the same period were selected as the control group.The serum GMP-140,CD63,CD62P levels,left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD)were compared between the two groups on the next day after study,and the serum GMP-140,CD63 and CD62P levels of patients with different cardiac function grades in the observation group were compared.The correlation between serum GMP-140,CD63,CD62P levels and cardiac function indexes was analyzed by Pearson correlation analysis.Results The serum GMP-140,CD63,CD62P levels,LVEDD,LVESD in the observation group were(53.18±6.22)ng/mL,(13.72±2.69)%,(14.89±2.77)%,(55.72±4.81)mm,(53.92±5.06)mm,which were significantly higher/larger than(27.83±3.17)ng/mL,(4.33±0.72)%,(3.56±0.61)%,(45.69±3.42)mm,(34.12±3.28)mm in the control group,and the LVEF was(45.82±4.75)%,which was significantly lower than(69.12±4.29)%of control group,with statistically significant differences(P<0.05)The serum GMP-140,CD63,and CD62P levels of patients with gradeⅣcardiac function in the observation group were(63.79±5.69)ng/mL,(18.35±2.62)%,and(20.12±2.71)%,which were significantly higher than(56.31±5.12)ng/mL,(14.28±2.05)%,(16.33±2.18)%of patients with gradeⅢcardiac function,(49.82±4.21)ng/mL,(11.52±2.17)%,(12.16±2.40)%of patients with gradeⅡcardiac function,and(39.64±3.76)ng/mL,(9.04±1.42)%,(8.76±1.68)%of patients with gradeⅠcardiac function;the level in patients with gradeⅢcardiac funct
