摘 要:目的分析四维超声时空关联成像(STIC)技术在胎儿先天性心脏病辅助诊断中的应用价值。方法选取2019年1月至2021年1月于我院产检的60例疑似胎儿先天性心脏病孕妇作为研究对象。以终止妊娠后尸检结果或胎儿出生后影像检查为金标准,比较二维超声和四维超声STIC技术对胎儿先天性心脏病的诊断价值。结果金标准检出42例胎儿先天性心脏病,二维超声检出34例胎儿先天性心脏病,四维超声STIC技术检出40例胎儿先天性心脏病。四维超声STIC技术对胎儿先天性心脏病的诊断灵敏度、特异度、准确度、阴性预测值均高于二维超声(P<0.05)。二维超声与金标准的一致性中等(Kappa值=0.552),四维超声STIC技术与金标准的一致性极高(Kappa值=0.828)。四维超声STIC技术的主动脉弓切面、腔静脉长轴切面、右心室流出道切面显示率高于二维超声(P<0.05)。结论四维超声STIC技术可提高胎儿先天性心脏病的诊断效能,为产前筛查提供可靠依据,对疾病的早期干预和预后改善意义重大。Objective To analyze the application value of four-dimensional ultrasound spatio-temporal imaging correlation(STIC)technology in auxiliary diagnosis of fetal congenital heart disease.Methods A total of 60 pregnant women with suspected fetal congenital heart disease examined in our hospital from January 2019 to January 2021 were selected as the research objects.Taking autopsy results after termination of pregnancy or fetal imaging examination after birth as the gold standard,the diagnostic value of two-dimensional ultrasound and four-dimensional ultrasound STIC technology in fetal congenital heart disease was compared.Results A total of 42 cases of fetal congenital heart disease were detected by gold standard,34 cases of fetal congenital heart disease were detected by two-dimensional ultrasound,and 40 cases of fetal congenital heart disease were detected by four-dimensional ultrasound STIC technology.The sensitivity,specificity,accuracy and negative predictive value of four-dimensional ultrasound STIC technology in diagnosis of fetal congenital heart disease were higher than those of two-dimensional ultrasound(P<0.05).The consistency between two-dimensional ultrasound and gold standard was medium(Kappa value=0.552),and the consistency between four-dimensional ultrasound STIC technology and gold standard was excellent(Kappa value=0.828).The display rates of aortic arch section,vena cava long axis section,right ventricular outflow tract section of four-dimensional ultrasound STIC technology were higher than those of two-dimensional ultrasound(P<0.05).Conclusion Four-dimensional ultrasound STIC technology can improve the diagnostic efficiency of fetal congenital heart disease,provide reliable basis for prenatal screening,and is of great significance for early intervention and prognosis improvement of the disease.
