腹腔镜下子宫全切术与开腹子宫全切术治疗妇科良性疾病的临床疗效比较

摘　　要：目的比较腹腔镜下子宫全切术与开腹子宫全切术治疗妇科良性疾病的临床疗效。方法选取66例妇科确诊的良性疾病患者为研究对象,按照就诊顺序编号(1~66号)对患者进行分组,1~33号予以开腹子宫全切术为开腹组,34~66号予以腹腔镜下子宫全切术为腹腔镜组,每组33例。比较两组手术相关指标、生活质量及术后并发症发生情况。结果腹腔镜组术中出血量(33.69±4.85)ml、术后排气时间(24.96±5.61)d、住院时间(5.64±1.62)d均优于开腹组(45.68±5.09)ml、(35.04±6.99)d、(7.96±1.38)d,差异显著(P<0.05)。两组手术时间比较差异无统计学意义(P>0.05)。术后3个月,腹腔镜组生活质量调查问卷(EORTC QLQ-C30)各维度评分均高于开腹组,差异显著(P<0.05)。腹腔镜组术后并发症发生率低于开腹组,差异显著(P<0.05)。结论与开腹子宫全切术比较,腹腔镜下子宫全切术治疗妇科良性疾病可减少术中出血,促进术后康复,且术后并发症发生率较低,提高患者术后生活质量。Objective To compare the clinical efficacy of laparoscopic hysterectomy and open hysterectomy in the treatment of gynecological benign diseases.Methods 66 patients with benign diseases diagnosed by gynecology department were selected as the research objects.The patients were divided into open hysterectomy group(1~33)and laparoscopic hysterectomy group(34~66),33 cases in each group.The operation related indexes,quality of life and postoperative complications were compared between the two groups.Results The intraoperative blood loss(33.69±4.85)ml,postoperative exhaust time(24.96±5.61)d,hospital stay(5.64±1.62)d in the laparoscopic group were better than those in the control group(45.68±5.09)ml,(35.04±6.99)d,(7.96±1.38)d.The difference was significant(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).At 3 months after operation,the scores of each dimension of EORTC QLQ-C30 in the laparoscopic group were higher than those in the open group,and the differences were significant(P<0.05).The incidence of postoperative complications in laparoscopic group was lower than that in open group,and the difference was significant(P<0.05).Conclusion Compared with total hysterectomy,laparoscopic total hysterectomy can reduce intraoperative bleeding,promote postoperative recovery,and improve the postoperative quality of life of patients with benign gynecological disease.
