负压腹壁引流管预防高危产妇剖宫产切口愈合不良及脂肪液化的应用

摘　　要：目的探析负压腹壁引流管应用于高危产妇剖宫产切口愈合不良及脂肪液化中的预防价值。方法选取33例剖宫产高危产妇,按照治疗策略不同分为对照组(20例,采用传统缝合)与研究组(13例,采用负压腹壁引流管)。对比两组术后康复时间、切口愈合情况、脂肪液化发生率及术后视觉模拟评分法(VAS)评分。结果研究组切口疼痛缓解时间、愈合时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。研究组切口愈合不良率15.38%、脂肪液化率7.69%均低于对照组50.00%、40.00%,差异有统计学意义(P<0.05)。两组术后1 d疼痛评分比较,差异无统计学意义(P>0.05);研究组术后3 d、2周疼痛评分(4.05±0.39)分、(1.29±0.40)分均低于对照组(5.18±0.44)分、(3.61±0.63)分,差异有统计学意义(P<0.05)。结论负压腹壁引流管应用于高危产妇剖宫产中效果显著,可促进切口快速愈合及疼痛缓解,降低切口愈合不良及脂肪液化风险发生率,值得临床推广与应用。Objective To explore the preventive value of negative pressure abdominal wall drainage tube used in highrisk puerperae in poor cesarean section incision healing and fat liquefaction.Methods 33 high-risk parturients of cesarean section were selected and divided into control group(20 cases,using traditional suture)and study group(13 cases,using negative pressure abdominal wall drainage tube)according to different drainage strategies.The postoperative recovery time,wound healing,incidence of fat liquefaction and postoperative visual analogue scale(VAS)score were compared between the two groups.Results The pain relief time,healing time and hospital stay in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The poor wound healing rate of 15.38%and fat liquefaction rate of 7.69%in the study group were lower than those in the control group(50.00%and 40.00%,respectively),and the differences were statistically significant(P<0.05).There was no significant difference in pain scores between the two groups at 1 day after operation(P>0.05).The pain scores of the study group at 3 days and 2 weeks after operation(4.05±0.39)and(1.29±0.40)were lower than those of the control group(5.18±0.44)and(3.61±0.63),and the differences were statistically significant(P<0.05).Conclusion The negative pressure abdominal drainage tube is effective in cesarean section for high-risk puerperae,can promote the rapid healing of incision and relieve pain,reduce the incidence of poor incision healing and the risk of fat liquefaction,and is worthy of clinical promotion and application.
