血栓弹力图指标与孕晚期血小板减少的相关性研究

摘　　要：目的探讨血栓弹力图(TEG)指标与孕晚期血小板减少的相关性研究。方法选取122例孕晚期孕妇作为研究对象,其中60例出现血小板减少的孕晚期孕妇作为研究组,62例孕晚期正常孕妇作为对照组。两组均行TEG及传统凝血四项检查。比较两组凝血四项指标水平、TEG参数;分析研究组血小板计数(PLT)水平与TEG参数的相关性。结果两组纤维蛋白原(FIB)、凝血酶原时间(PT)、凝血酶时间(TT)及活化部分凝血活酶时间(APTT)水平比较,差异无统计学意义(P>0.05)。研究组TEG参数中凝血反应时间(R)和凝固角(α角)与对照组比较,差异无统计学意义(P>0.05);研究组TEG参数中凝血综合指数(CI)(3.24±0.48)、凝固时间(K值)(1.01±0.21)min、最大振幅(MA)(62.19±5.49)mm优于对照组(4.25±1.05)、(0.84±0.16)min、(65.79±5.14)mm,差异有统计学意义(P<0.05)。研究组PLT水平与TEG参数中R及K呈负相关(r=-0.135、-0.564,P<0.05),与CI、MA及α角呈正相关(r=0.595、0.642、0.613,P<0.05)。结论TEG可在一定程度上反映血小板功能,可为孕晚期孕妇就诊提供数据参考。Objective To investigate the correlation between thromboelastography(TEG)index and thrombocytopenia in the third trimester.Methods 122 pregnant women in the third trimester were selected as the research objects,including 60 pregnant women with thrombocytopenia as the study group,62 pregnant women in the third trimester as the control group.TEG and traditional coagulation tests were performed in both groups.The levels of four coagulation indexes and TEG parameters were compared between the two groups.The correlation between platelet count(PLT)and TEG parameters was analyzed.Results There were no significant differences in fibrinogen(FIB),prothrombin time(PT),thrombin time(TT)and activated partial thromboplastin time(APTT)between the two groups(P>0.05).There were no significant differences in coagulation reaction time(R)and coagulation Angle(αAngle)between the study group and the control group(P>0.05).The coagulation composite index(CI)(3.24±0.48),coagulation time(K value)(1.01±0.21)min,maximum amplitude(MA)(62.19±5.49)mm in the study group were better than those in the control group(4.25±1.05),(0.84±0.16)min,(65.79±5.14)mm,the difference was statistically significant(P<0.05).In the study group,PLT level was negatively correlated with R and K of TEG parameters(r=-0.135,-0.564,P<0.05),and positively correlated with CI,MA andαAngle(r=0.595,0.642,0.613,P<0.05).Conclusion TEG can reflect platelet function to a certain extent,which can provide data reference for pregnant women in the third trimester.
