多根多处肋骨骨折患者行胸腔闭式引流术的整体护理体会

摘　　要：目的分析胸腔闭式引流术治疗多根肋骨骨折患者时应用整体护理时取得的临床效果。方法2018年8月至2019年8月收治于我院的40例肋骨骨折患者以随机数字表法分为观察组和对照组,平均20例。患者均以胸腔闭式引流术治疗,分别按照整体护理和常规护理对两组患者开展不同的护理干预,并对经过不同护理之后的恢复所需时间、疼痛程度、焦虑和抑郁情绪、睡眠条件、生活质量、骨折并发症与患者对护理的满意度等进行统计分析和对比。结果①观察组在术后首次下床活动所需时间、住院所需时间、骨折愈合所需时间上都比对照组明显更短(P<0.05)。②护理之前,观察组和对照组在疼痛、负面心理和睡眠质量上差异无统计学意义(P>0.05),护理之后,观察组的疼痛明显减轻,负面心理和睡眠质量都有明显的调整,与对照组相比差异有统计学意义(P<0.05)。③护理之前观察组和对照组患者在生活质量上差异无统计学意义(P>0.05),经过护理之后观察组的生活质量改善比对照组更好(P<0.05)。④观察组(2例,占10.00%)护理之后并发症出现概率比对照组(11例,占55.00%)低(P<0.05)。⑤观察组(19例,占95.00%)患者对满意度的认可比对照组(14例,占70.00%)高(P<0.05)。结论通过胸腔闭式引流术治疗的多根多处肋骨骨折人群在护理时实施整体护理可以显著缩短患者康复所需时间,减缓机体的疼痛和心理上的不良情绪,有助于促进睡眠,同时改善患者的生活质量,减少并发症的概率,使患者对护理更加认可和满意。Objective To analyze the clinical effect of holistic nursing in the treatment of patients with multiple rib fractures by closed thoracic drainage.Methods From August 2018 to August 2019,40 patients with rib fractures in our hospital were randomly divided into observation group and control group,with an average of 20 cases.The patients were treated with closed thoracic drainage,and different nursing interventions were carried out according to holistic nursing and routine nursing for the two groups of patients,and the recovery time,pain degree,anxiety and depression,sleep conditions,quality of life,fracture complications and patient satisfaction with nursing after different nursing were statistically analyzed and compared.Results(1)The observation group was significantly shorter than the control group in the time of first ambulation,hospitalization and fracture healing(P<0.05).(2)Before nursing,the observation group and the control group did not show significant deviation in pain,negative psychology and sleep quality(P>0.05).After nursing,the pain of the observation group was significantly alleviated,and the negative psychology and sleep quality were significantly adjusted,which was statistically different from the control group(P<0.05).(3)Before nursing,there was no significant difference in quality of life between the observation group and the control group(P>0.05),but after nursing,the improvement of quality of life in the observation group was better than that in the control group(P<0.05).(4)Observation group(2 cases,accounting for 10.00%)The incidence of complications after nursing was lower than that in the control group(11 cases,accounting for 55.00%)(P<0.05).(5)The satisfaction of patients in the observation group(19 cases,accounting for 95.00%)was higher than that in the control group(14 cases,70.00%)(P<0.05).Conclusion The implementation of holistic nursing care for patients with multiple rib fractures treated by closed thoracic drainage can significantly shorten the time required for rehabilitation,allev
