滋阴颗粒剂方治疗女童特发性性早熟的临床观察

摘　　要：目的分析滋阴颗粒剂方治疗女童特发性性早熟的临床疗效,观察其对患儿的临床症状、体征、辅助检查及生长发育的影响。方法选取2021年1月至2022年1月就诊于绍兴市妇幼保健院小儿内分泌科的特发性性早熟女童90例,采用随机数字表法分为大补阴丸组(阳性对照组)和滋阴颗粒组(治疗组),每组各45例。滋阴颗粒组给予滋阴颗粒剂方治疗,大补阴丸组给予大补阴丸治疗。比较两组女童治疗前(0个月)、治疗后(3个月、6个月)的临床症状、体征、卵泡刺激素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)、雌二醇(estradiol,E_(2))和泌乳素(prolactin,PRL)水平、子宫卵巢容积、中医症候评分(traditional Chinese medicine symtom score,TCMSS)变化,评估滋阴颗粒剂方及大补阴丸的疗效。结果治疗前,两组女童的年龄、体质量、身高、Tanner分期、血清基础性激素值、骨龄等比较,差异均无统计学意义(P>0.05),治疗6个月后,两组的乳核指数比治疗前有所减少,差异有统计学意义(P<0.01);两组的子宫容积与治疗前比较均减小,差异均有统计学意义(P<0.05);两组的FSH、LH、E2值、卵巢容积与治疗前比较均降低,差异均有统计学意义(P<0.01)。结论滋阴颗粒剂方能有效改善女童特发性性早熟的临床症状,降低FSH、LH、E2值,缩小子宫容积、卵巢容积并且操作方便,依从性强,具有临床应用价值。Objective The purpose of this study was to evaluate the clinical efficacy of Ziyin Keli formula in the treatment of idiopathic precocious puberty in girls to that of Dabuyin Wan.The effects on clinical symptoms,signs,auxiliary examinations,and the children's growth and development were studied.Methods According to the medication used,a total of 90 girls with idiopathic precocious puberty who visited the Department of Paediatric Endocrinology,Shaoxing City Women and Children Hospital between January 2021 and January 2022 were divided into the Dabuyin Wan group(positive control group)and the Ziyin Keli group(treatment group),with 45 cases in each group.To assess the efficacy of the Ziyin Keli formula and the Dabuyin Wan,the clinical symptoms,signs,changes in follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2))and prolactin(PRL),uterine ovarian volume,and traditional chinese medicine symtom score(TCMSS)of the children in the two groups were analyzed before(0 month),after 3 months,and after 6 months of treatment.Results Before treatment,there was no statistical difference in age,weight,height,Tanner stage,serum basal sex hormone values and bone age between the two groups of girls(P>0.05).After 6 months of treatment,the mammary nucleus index decreased in both groups compared to pre–treatment,with a statistically significant difference(P<0.01);uterine volume decreased in both groups compared to pre–treatment,with a statistically significant difference(P<0.05),and FSH,LH,E2 values and ovarian volume decreased in both groups compared to pre–treatment,with a statistically significant difference(P<0.01).Conclusion Ziyin Keli may effectively relieve clinical symptoms of idiopathic precocious puberty in girls,reduce FSH,LH,and E2 levels, shrink uterine and ovarian volume, and is simple to use and follow, therefore it has clinical application.
