摘　　要：目的 分析加速康复外科(ERAS)下的改良单切口手术治疗结核性外穿性脓胸的临床疗效。方法 2020年1月至2021年3月我院手术治疗的60例结核性外穿性脓胸患者,对照组(n=30)行传统手术,观察组(n=30)行基于ERAS理念的改良单切口手术。比较两组治疗效果、手术一般指标与预后情况、术后肺通气功能指标及并发症。结果 观察组总有效率高于对照组,手术、恢复及肺通气指标均优于对照组,并发症发生率、疾病复发率均低于对照组(P<0.05)。结论 基于ERAS理念的改良单切口治疗本研究观察组研究对象效果良好,一般手术指标得到有效改善、疼痛减轻、带管时间短、住院时间短,值得推广应用。Objective To analyze the clinical curative effect of modified single-incision based on the concept of enhanced recovery after surgery(ERAS) in the treatment of tuberculous perforated empyema.Methods A total of 60 patients with tuberculous perforated empyema undergoing surgical treatment in our hospital from January 2020 to March 2021 were selected. According to different surgical methods, the patients were divided into a control group and an observation group, 30 in each group. The control group was treated with traditional surgery and the observation group was treated with modified single-incision surgery based on ERAS concept. The curative effect, surgical indexes, prognosis, indexes of postoperative pulmonary ventilation function and complications were compared between the two groups.Results The total response rate in the observation group was higher than that in the control group. The indexes of operation, recovery and lung ventilation in the observation group were better than those of the control group. The incidence of complications and disease recurrence rate in the observation group were lower than those in the control group(P<0.05). Conclusions The curative effect of modified single-incision surgery based on ERAS concept is good in the observation group. It can effectively improve the surgical indexes, relieve the pain, and shorten the intubation time and hospitalization time. It is worthy of population and application.
