摘 要:目的 分析抗角蛋白抗体(AKA)、抗核周因子(APF)、葡萄糖-磷酸异构酶(GPI)联合检测在类风湿关节炎(RA)早期诊断中的应用价值。方法 选取2019年3月至2021年3月我院收治的106例RA患者(RA组),同期非RA自身免疫性疾病患者91例(非RA组)。比较不同人群、不同分期、活动期与非活动期患者中AKA、APF、GPI阳性率;分析AKA、APF、GPI单独及联合检测对类风湿关节炎的诊断价值。结果 RA组AKA、APF、GPI阳性率高于非RA组(P<0.05);活动期AKA、APF、GPI阳性率高于非活动期(P<0.05);重度组AKA、APF、GPI阳性率高于轻度组(P<0.05);AKA+APF+GPI检测RA诊断灵敏度、特异度、阳性预测值、阴性预测值分别为97.16%、93.41%、94.50%、96.59%,高于三因子单独检测(P<0.05)。结论 AKA、APF、GPI联合检测可提高对RA诊断效能,可为临床提供更全面参考信息,对临床诊疗有一定的指导意义。Objective To analyze the application value of combined detection of anti-keratin antibody(AKA), antiperinuclear factor(APF), and glucose-phosphate isomerase(GPI) in the clinical diagnosis of rheumatoid arthritis(RA).Methods One hundred and six RA patients admitted to our hospital from March 2019 to March 2021 were selected as a RA group. Another 91 with non-RA autoimmune diseases during the same time were selected as a non-RA group. The positive rates of AKA, APF, and GPI in patients with different populations, different stages, active and inactive periods were compared. The diagnostic value of AKA, APF and GPI alone and in combination for rheumatoid arthritis was analyzed.Results The positive rates of AKA, APF, and GPI in the RA group were higher than those in the non-RA group(P<0.05). The positive rates of AKA, APF, and GPI in the active period were higher than those in the inactive period(P<0.05). The positive rates of AKA, APF, and GPI in the severe group were higher than those in the mild group(P<0.05). The sensitivity, specificity, positive predictive value, and negative predictive value of AKA+APF+GPI for detecting RA were 97.16%, 93.41%, 94.50%, 96.59%, respectively, which were significantly higher than the three factors tested alone(P<0.05).Conclusions The combined detection of AKA, APF and GPI can improve the efficacy of early diagnosis of RA. It can provide more comprehensive reference information for the clinic, and has certain guiding significance for clinical diagnosis and treatment.
