摘 要:目的 观察抗痨合剂对老年肺结核患者外周血B细胞的影响。方法 选取2019年1月至2020年12月丽水市中医院收治的老年肺结核患者249例纳入研究,采用随机数字表法将其分为对照组(123例)和观察组(126例)。对照组患者予常规西药抗结核治疗,观察组患者在对照组基础上口服抗痨合剂治疗。比较两组患者治疗前后CD19^(+)B细胞百分比和绝对计数、B细胞亚群百分比、中医证候积分和治疗转归。结果 观察组患者治疗后的中医证候积分显著低于对照组(P<0.05),观察组患者的治疗成功率显著高于对照组(P<0.05)。治疗2个月后,观察组患者的外周血B细胞百分比和绝对计数、CD19^(+)CD1d^(-)CD5^(-)B细胞和CD19^(+)CD1d^(-)CD5^(+)B细胞的表达水平均显著高于对照组(P<0.05),CD19^(+)CD1d^(+)CD5~-B细胞和CD19^(+)CD1d^(+)CD5^(+)B细胞的表达水平均显著低于对照组(P<0.05)。结论 抗痨合剂可通过增加老年肺结核患者外周血B细胞的百分比及绝对计数,改善B细胞亚群稳态,从而发挥抗结核治疗作用。Objective To observe the effect of anti-tuberculosis decoction on B cells in peripheral blood of elderly patients with pulmonary tuberculosis.Methods From January 2019 to December 2020,249 elderly pulmonary tuberculosis patients admitted to Lishui Hospital of Chinese Medicine were selected into study and divided into control group(123 cases)and observation group(126 cases)by random number table.Patients in control group were treated with conventional western medicine for anti-tuberculosis.Patients in observation group were treated with anti-tuberculosis decoction on the basis of the control group.The percentage of CD19^(+)B cells,the absolute B cell count,the percentage of B cell subsets,TCM syndrome scores and treatment outcomes were compared between the two groups before and after treatment.Results The TCM syndrome scores in observation group after treatment were significantly lower than those in control group(P<0.05).The treatment success rate in observation group was significantly higher than that in control group(P<0.05).After 2 months of treatment,the percentage and absolute count of peripheral blood B cells,the expression levels of CD19^(+)CD1d^(-)CD5^(-)B cells and CD19^(+)CD1d-CD5^(+)B cells in observation group were significantly higher than those in control group(P<0.05),and the expression levels of CD19^(+)CD1d^(+)CD5-B cells and CD19^(+)CD1d^(+)CD5^(+)B cells were significantly lower than those in control group(P<0.05).Conclusion Anti-tuberculosis decoction can improve the homeostasis of B cell subsets by increasing the percentage and absolute count of peripheral blood B cells in elderly patients with pulmonary tuberculosis,so as to play an anti-tuberculosis therapeutic role.
