摘 要:目的:探讨血清C反应蛋白(CRP)、红细胞沉降率检测在川崎病早期诊断中的价值。方法:选取2018年1月-2021年10月贵州航天医院收治的川崎病患儿40例作为观察组,根据患儿的临床表现又分为典型川崎病组(32例)和非典型川崎病组(8例)。另外选取同期40例发热>3 d的急性扁桃体炎、支气管肺炎患儿作为对照组。比较各组血清CRP、红细胞沉降率变化情况,分析血清CRP、红细胞沉降率测定结果及联合诊断的灵敏度、特异度和准确率。结果:观察组患儿血清CRP、红细胞沉降率相较于对照组呈明显升高趋势,差异有统计学意义(P<0.05)。非典型川崎病组患儿血清CRP、红细胞沉降率与典型川崎病组比较均明显升高(P<0.05)。血清CRP与红细胞沉降率联合检测的灵敏度、特异度和准确率均高于血清CRP、红细胞沉降率单独检测,差异有统计学意义(P<0.05)。结论:血清CRP、红细胞沉降率升高与川崎病具有密切联系,对川崎病早期诊断具有重要价值,而血清CRP、红细胞沉降率联合检测可进一步提升诊断准确率,值得临床推广。Objective:To explore the value of serum C-reactive protein(CRP)and erythrocyte sedimentation rate in the early diagnosis of Kawasaki disease.Methods:A total of 40 children with Kawasaki disease in Guizhou Aerospace Hospital from January 2018 to October 2021 were selected as the observation group.The children were divided into typical Kawasaki disease group(n=32)and atypical Kawasaki disease group(n=8)according to their clinical manifestations.In addition,40 children with acute tonsillitis and bronchial pneumonia who had fever for more than 3 days were selected as the control group during the same period.The changes of serum CRP and erythrocyte sedimentation rate in each group were compared.The determination results of serum CRP and erythrocyte sedimentation rate,as well as the sensitivity,specificity and accuracy of the combined diagnosis were analyzed.Results:The serum CRP and erythrocyte sedimentation rate of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).The serum CRP and erythrocyte sedimentation rate of children in typical Kawasaki disease group were significantly increased compared with those in atypical Kawasaki disease group(P<0.05).The sensitivity,specificity and accuracy of the combined detection for serum CRP and erythrocyte sedimentation rate were higher than those of the individual detections,with statistically significant differences(P<0.05).Conclusion:The increase in serum CRP and erythrocyte sedimentation rate is closely related to Kawasaki disease and is of great value for the early diagnosis of Kawasaki disease.The combined diagnosis of serum CRP and erythrocyte sedimentation rate can further improve the diagnostic accuracy and is worthy of promotion.
